摘要
目的:研究喉鳞癌(laryngeal squamous cell carcinoma,LSCC)中骨桥蛋白(osteopontin,OPN)的表达状况及其与临床病理特征之间的关系,阐明骨桥蛋白在喉鳞癌侵袭和转移过程中的作用,并为临床治疗及防治喉鳞癌侵袭、转移提供理论依据和新的思路。方法:收集40例接受手术治疗并经病理诊断证实为原发性喉鳞癌(LSCC)患者病理组织石蜡标本作为试验组,另取40例同期手术切除经病理证实为癌旁阴性喉黏膜组织石蜡标本作为对照组。所有石蜡标本5μm连续切片2张,其中1张行HE染色,证实其为何种病理组织,另外1张用免疫组织化学染色SP法检测OPN的表达。结果:OPN在喉鳞癌组中的阳性表达率为75.00%;在癌旁阴性喉黏膜组中阳性表达率为5.00%;表达强度差异具有统计学意义(P<0.05)。OPN表达与喉鳞癌原发部位无明显关系(P>0.05),而与T分期呈正相关(P<0.05);有局部淋巴结转移组高于无局部淋巴结转移组,其比较差异具有统计学意义(P<0.05);与喉鳞癌细胞分化程度呈负相关(P<0.05)。结论:OPN在喉鳞癌中高表达,与有无局部淋巴结转移、病理学分级、T分期等有关,与喉鳞癌原发部位无关。高表达的OPN在喉鳞癌的发生、发展、侵袭和转移中发挥作用,同时OPN可作为评估喉鳞癌进展、转移的有价值的指标。因此,推测阻断或抑制其表达和活性,干预传导通路中的某些关键位点,可能会成为有效治疗喉鳞癌的新思路。
Objective:To investigate the expression of osteopontin and its correlation with clinicopathologic features of LSCC (laryngeal squamous cell carcinoma),to elucidate its role in tumor invasion and distant metastasis, and also to provide new idea and theoretical evidence for clinical treatment of LSCC and prevention of its metastasis or palindromia. Methods:40 cases of LSCC pathological tissues were collected as test group;forty samples of adjacent non-cancerous tissues as control group. All the pathological paraffin tissues were undergone serial section and two sections about 5μm were obtained,one for HE dying to confirm what tissue it was,another was assayed using immunohistochemical SP method for detection of OPN expression level. Positive expression ratio of OPN in LSCC and adjacent non-cancerous tissue was calculated. Results:The target gene of OPN was detected in 30 of 40 tumor tissues (75%), and 2 of 40 adjacent non-cancerous tissues (5%),the difference was significant (P〈0.05);no significantly difference was found among the expression of OPN in different primary tumor locations (P〉0.05);there was significant statistical difference between the early clinical stages (T1-2) and the advanced stages (T3-4) ;the moderate-poorly differentiated tumor cell showed a higher OPN expression than the highly differentiated tumor cell (P〈0.05)and the OPN expression in the lymph node metastasis group was higher than in the non-metastasis group (P〈0.05). Conclusions: ① OPN was over expressed in LSCC than in nonmalignant tissues. The up-regulated OPN correlates with node metastasis,clinical stages,and pathological grading, but has no relationship with the primary tumor locations. Over expression of OPN plays a role in genesis, development infiltration and metastasis of LSCC. ②As an index of infiltration and metastasis of LSCC,OPN is deemed to be the good objectives of therapy,so we presume that the blockage and inhibition of activity or expression of OPN can be effective
