摘要
目的探讨视黄醇结合蛋白-4(RBP-4)水平与肥胖病人体重指数、胰岛素抵抗的关系。方法选取我院(2009年3月–2010年3月)内分泌科住院病人71例,其中肥胖36例(男孩19例,女孩17例),正常体重35例。测量体重、身高、腰围、血压等,并测定其血清RBP-4、空腹血糖及胰岛素水平,并计算HOMA-IR,分析血清RBP-4水平与体重指数、空腹血糖及胰岛素抵抗指数HOMA-IR之间的相关性。结果相对于正常体重儿童,肥胖儿童的血清RBP4水平显著升高(P<0.01),且肥胖程度越高,RBP4水平越高,而减重后RBP4水平显著下降(P<0.01)。青春期儿童血清RBP4水平升高,血清RBP4水平无性别差异。多元回归分析剔除年龄、性别、青春发育阶段影响后,RBP4与肥胖程度、体重指数、胰岛素抵抗指数HOMA-IR有显著相关。结论肥胖儿童血清RBP4水平升高,其水平与肥胖程度、体重指数BMI、胰岛素抵抗指数HOMA-IR显著相关。
Objective To explore retinol binding protein -4 (RBP-4) levels and obesity patient body mass index, insulin resistance relationship. Methods Choose 71 inpatients of endocrinology department of JiangXi children's hospital during Mar 2009 to Mar 2010.There were 36 obese children (male 19,female 17),35 normal body weight. Measuring weight,height, waistline, blood pressure, and determine the serum RBP-4, fasting blood sugar, blood fat and insulin levels, and calculate insulin resistance index homestasis model assessment (HOMA-IR), analysis the correlation between RBP-4 levels, BMI,fasting glucose and HOMA- IR. Results Compared with the normal body weight children, Obese children had significantly (P〈0.01) higher RBP4 concentra- tions, And the higher the degree of obesity, RBP4 level is higher, while after the weight loss RBP4 levels decreased significantly(P 〈0.01). Pubertal children had higher serum RBP4 concentrations , and which had no gender differences. In multiple linear regression analyses adjusted to age, gender, and pubertal stage, RBP4 was significantly correlated to the degree of obesity.BMI and HOMA-IR. Conclusion Obese children had a higher serum RBP4 concentrations,RBP4 levels had Positive correlation with BMI and HOMA-IR.
