摘要
目的探讨内镜下取石网篮及球囊治疗胆总管结石的疗效。方法分析宣城市人民医院胆总管结石116例患者资料,随机分为两组,治疗组(n=61)使用取石网篮及球囊取石,对照组(n=55)单纯取石网篮取石,观察两组的治愈率、近期并发症、远期结石复发情况。结果治疗组成功取石58例,对照组成功取石51例,两组取石成功率分别为95.08%和92.73%,差异无统计学意义(P>0.05);两组近期并发症(穿孔各为0,胰腺炎分别为4例和3例,出血分别为2例和3例)发生率分别为9.8%和10.91%,差异无统计学意义(P>0.05);远期结石复发分别为1例和8例,复发率分别为1.72%和15.69%,差异有统计学意义(P<0.05)。对照组单纯网篮取石成功使用网篮108次;治疗组取石成功58例,网篮使用38次,球囊使用40次。7例未能取石,行置入塑料或金属支架治疗。结论内镜下取石网篮及球囊治疗胆总管结石具有疗效佳、成功率高、并发症少、复发率低等优点。
Objective To study the curative effect of basket and balloon stone extraction on common bile duct calculi.Methods The clinical data of 116 patients with common bile duct calculi from Xuancheng people＇s hospital were analyzed retrospectively.All patients were randomly divided into two groups.The treatment group（n=61） underwent the basket or balloon stone extraction,while the control group（n=55） underwent the simply basket stone extraction.The cure rate,short-term complications and long-term recurrence of calculi were observed and compared.Results The stones of 58 cases in the treatment group and of 51 cases in the control group were removed successfully,and the success rates were 95.08% and 92.73%,respectively（P0.05）;the incidence of short-term complications（perforation,pancreatitis and hemorrhage） were 9.8% in the treatment group and 10.91% in the control group,respectively（P0.05）;The rate of long-term recurrence of calculi were 1.72% and 15.69%,respectively（P0.05）.The control group made use of basket 108 times,the treatment group made use of basket 38 times and balloon 40 times.7 cases failed to extract stones,and convert to biliary stent placement by endoscope.Conclusion Balloon or basket stone extraction under endoscope for common bile duct calculi is with the advantages of better curative effect,higher success rate,fewer complications and lower recurrence rate.
