3T磁共振成像相位对比法评价法洛四联症根治术后患者右心室舒张功能

Evaluation of phase contrast magnetic resonance imaging at 3.0 Tesla on right ventricular diastolic function in patients with repaired tetralogy of Fallot

摘要 目的通过3T相位对比磁共振成像(phase contrast-magnetic resonance imaging,PC-MRI)对法洛四联症(tetralogy of Fallot,TOF)术后随访患者三尖瓣血流评估,与超声心动图结果进行对照研究,探讨PC-MRI评价TOF术后患者右心室舒张功能的准确性。方法27例进行了超声心动图及三尖瓣PC-MRI的TOF随访患者纳入研究。PC-MRI测量得到三尖瓣E波峰值流速(Evelocity)、A波峰值流速(Avelocity);E波峰值流率(Eflow)、A波峰值流率(Aflow)。超声心动图测量得到E波和A波峰值流速(分别表示为E、A)。Evelocity/Avelocity及Eflow/Aflow比值分别与超声心动图测量E/A比值进行相关分析及配对样本t检验。按照E/A>1(无舒张功能障碍)及E/A≤1(有舒张功能障碍)分为两组,比较PC-MRI两种测量方法与超声心动图测量结果符合程度。结果PC-MRI测量的Evelocity/Avelocity,Eflow/Aflow比值与超声心动图测量E/A比值具有正相关性(r=0.560,P=0.002;r=0.542,P=0.003)。PC-MRI测量结果 Evelocity/Avelocity及Eflow/Aflow比值与超声心动图测量E/A比值相比,差异无统计学意义(1.29 vs.1.41,t=1.624,P=0.116;1.39 vs.1.41,t=0.182,P=0.857)。判断有、无舒张功能障碍,PC-MRI峰值流速比值(Evelocity/Avelocity)、峰值流率比值(Eflow/Aflow)与超声心动图符合程度较好(K=0.697,P<0.001;K=0.571,P=0.003)。结论PC-MRI测量的三尖瓣峰值流速比值及峰值流率比值能够正确判断右心室舒张功能。 Objectives To compare E- and A- wave flow patterns across the tricuspid valve in patients with repaired tetralogy of Fallot (TOF) using phase contrast-magnetic resonance imaging (PC-MRI) with data provided by echocardiography, and to evaluate the accuracy of right ventricular diastolic function by PC-MRI at 3.0 tesla. Methods This study included 27 patients with repaired TOF who accepted echocardiography and PC-MRI examination. Tricuspid valve E velocity (Evelocity), A velocity (Avelocity), E-flow (Eflow) and A-flow (Atlow) were measured by PC-MRI; E velocity (E) and A velocity (A) were also identified by echocardiography. The ratios of Evelocity/Avelocity, Eilow/ Allow and E/A were calculated. Spearman rank correlation analysis and paired sample t test were performed for comparison. According to E/A, patients were categorized into E/A〉1(normal diastolic function) and E/A ≤1(diastolic dysfunction) groups,and the consistency degree between PC-MRI and echocardiography was compared. Results The ratios of Evelocity/Avelocity and Eflow/Aflow measured by PC-MRI were positively correlated with E/A measured by echocardiography (r=0.560,P=0.002; r=0.542,P=0.003), but there were no significant differences (1.29 vs. 1.41, t=1.624, P=0.116; 1.39 vs. 1.41, t=O.182,P=0.857). The ratios of Evelocity/Avelocity and Eflow/Aflow were correlated well with E/A measured by echocardiography (K=0.697, P〈0.001 ; K=0.571, P=0.003) for diagnosis of diastolic dysfunction. Conclusions The ratios of Evelocity/Avelocity and Eflow/Aflow measured by PC-MRI at 3.0 Tesla can evaluate right ventricle diastolic function correctly.
