Effect of Different Detection Algorithms on Inappropriate Shocks Caused by Single Chamber Implantable Cardioverter Defibrillators

摘要 目的 探讨单腔植入型心律转复除颤器(ICD) 不同识别参数设置对不恰当放电的影响.方法 A组入选25例2007年至2008年植入单腔ICD的患者,术后经验设置室性心律失常与室上性心动过速鉴别参数.B组入选25例2009年至2010年植入单腔ICD的患者,术后开启稳定性、突发性和形态学标准.每例随访1年.观察两组室上性心动过速正确识别率和不恰当放电率.结果 与A组相比,B组对室上性心动过速正确识别率较高(68.0%、28.0%),室上性心动过速放电率较低(24.0%、76.0%),不恰当放电所致住院率较低(4.0%、20.0%).A组不恰当放电引发室性心动过速2例.结论 术后启用三项鉴别参数能减少ICD不恰当放电. Objective To explore the effect of different detection algorithms on inappropriate shocks caused by single chamber implantable cardioverter defibrillators (ICD). Methods 25 patients implanted with single chamber ICD during 2007-2008 were included in group A and another 25 patients implanted with single chamber ICD during 2009-2010 were included in group B. The parameters identifying ventricular and supraventricular tachycardia(SVT) were set based on per- sonal experiences in group A. While the criteria such as stability, sudden-onset and morphology were implemented after the procedure in group B. All the patients were followed up for a year. The diagnosis accuracy of SVT and inappropriate shocks were analyzed. Results The percentage of correct diagnoses of SVT was higher in group B than in group A (68.0% vs.28.0%). Inappropriate shocks and hospitalization for inappropriate shocks were less in group B (20.0% vs. 84.0%) than in group A (24.0% vs.76.0%). Ventricular tachycardia was induced by inappropriate shocks in 2 cases of group A. Conclusion The application of stability, sudden-onset and morphology after procedure may reduce inappro- priate shocks caused by ICD.
