电灼伤患者血栓前状态的临床研究

摘要 目的 研究电灼伤患者的血栓前状态。方法  检测26例电灼伤患者血浆中血小板P-选择素、内皮素(ET- 1)、抗凝血酶Ⅲ活性(ATⅢ:A)和凝血因子 Ⅱ、 Ⅴ、 Ⅷ、 Ⅸ促凝活性,并加以分析。 结果 在电灼伤早期,P-选择素和ET-1显著增高;ATⅢ:A先下降,然后在1周时恢复至正常水平;凝血因子Ⅱ、Ⅴ、Ⅷ、Ⅸ促凝活性变化不明显,但1周后可有不同程度的降低。结论 电灼伤激活了血小板和凝血因子,致使病人处于血栓前状态,或局部有血栓形成。 Objective To study the prethrombotic state in the patients of electric burn injury. Methods The plasma levels of platelet P -- selectin, endothelin-- 1 (ET-- 1), antithrombin Ⅲ activiny(ATⅢ:A), as well as a series of coagulating factors, including factor Ⅱ,Ⅴ,Ⅷ and Ⅸ activity were measured in twenty-- six patients of electric burn injury. Results Both P-- selectin and ET -- 1 increased significantly in the early phase of electric burn injury (P<0.01), while AT -- Ⅲ:A decreased and factor Ⅱ, Ⅴ,Ⅷ and Ⅸ activity were not altered. However, after one week, AT -- Ⅲ: A returned to normal, and the other coagulating factors some what decreased. Conclusion There exist activation of platelets and coagulating procedures, resulting in a prethrombotic state and local thrombosis formation in the patients of electric burn injury.
