摘要
目的探讨老年人维生素D水平与骨密度的关系。方法选择2010年7月至2011年5月在我院干部病房住院的老年患者118例,年龄60~92岁,平均(77.7±11.2)岁。采用双能X线骨密度仪(DEXA)测定1~4腰椎(L1-4)和股骨颈骨密度,据骨密度分为骨质疏松组和非骨质疏松组,测定血清25-羟基维生素D3[25(OH)D3]水平,收集身高、体质量、年龄等资料,计算体质指数,并进行相关性分析。结果骨质疏松组年龄(81.6±5.6)岁、BMI(22.5±4.0)kg/m^2,与非骨质疏松组(79.4±6.9)岁、(24.1±4.2)kg/m^2比较,差异无统计学意义(t值分别为1.80、-2.01,均P〉0.01);骨质疏松组血清25(OH)D3为(21.6±10.3)nmol/L,低于非骨质疏松组(32.0±13.8)nmol/L,差异有统计学意义(t=-4.20,P〈0.01);血清25(OH)D3≤50nmol/L者,骨质疏松组为95.3%(41/43),非骨质疏松组为81.3%(61/75),差异有统计学意义(Х^2=4.58,P〈0.05)。进一步直线相关分析结果显示,血清25(OH)D3水平与股骨颈骨密度呈正相关(r=0.22,P〈0.05),但尚不能认为血清25(OH)D3水平与L1-4骨密度存在直线相关(r=0.18,P〉0.05)。结论老年人维生素D营养状况与骨密度有关,直线相关分析结果显示血清25(OH)D。水平与股骨颈骨密度呈正相关。
Objective To study the association between vitamin D status and bone mineral density(BMD) in aged people. Methods A total of 118 patients admitted in cadre department aged (77.7±11.2) years were selected from July 2010 to May 2011. The BMD at lumbar spine (L14 ) and femoral neck was measured by dual-energy X-ray absorptiometry (DEXA). According to BMD, patients were divided into two groups: osteoporosis and nonosteoporosis group. The data of serum 25-hydroxy vitamin D3 [25 (OH)D33 concentration, height, weight and age of patients were collected at the same time. Results The age and body mass index (BMI) in osteoporosis group were (81.6± 5.6) years and (22.5±4.0)kg/m^2, while (79.4±6.9)years and (24.1±4.2)kg/m^2 in nonosteoporosis group (t= 1.80 and --2.01,P〈0.05). The concentrations of serum 25(OH)D3 in two groups were(21.6±10.3)nmol/L and (32.0±13.8) nmol/L, respectively(t-- 4.20, P〈0.01). And there were 95.3% (41/43)and 81.3%(61/75)of patients whose serum 25(OH)D3 level were 450 nmol/L in osteoporosis and nonosteoporosis group, respeetively(Х^2 : 4.58, P〈0. 05). Furthermore, the 25(OH)D31evel was positively correlated with BMD at femoral neck(r 0. 22,P〈0. 05), but not correlated with BMD at L14 (r:0.18,P〈0.05). Conclusions Vitamin D status is correlated with BMD at femoral neck in aged people.
