Krackow ANASTOMOSIS METHOD TO REPAIR THE CLOSED RUPTURE OF ACHILLES TENDON

摘要 目的探讨Krackow缝合法治疗闭合性跟腱断裂的效果。方法回顾2008年1月—2011年12月闭合性跟腱断裂的患者25例,按采用的手术缝合方法不同分为Krackow法组及Bunnell法组,术中使用抗菌薇乔线缝合跟腱断端,术后给予石膏外固定,逐渐功能锻练,随访2组的术后并发症、功能恢复情况及患者对术后恢复的满意程度。结果 25例患者均获得随访,采用Arner-Lindholm疗效评定标准对2组种手术术后功能恢复情况进行评定,总优良率为96.2%,Krackow法组优良率100.0%,Bunnell法组优良率92.3%,2组术后早期(4个月)功能恢复分级差异有统计学意义(P=0.02),患者术后满意率差异有统计学意义(P<0.05),而2组术后并发症发生率差异无统计学意义(P>0.05)。结论运用Krackow法修复闭合性跟腱断裂,利于跟腱早期功能恢复,术后功能恢复良好,患者满意率高。 Objective To investigate the result of the Krackow anastomosis method to repair the closed rupture of Achilles tendon. Methods Twenty - five cases of Achilles tendon rupture from Jan. 2008 to Dec. 2011were reviewed. All the patients were divided into Krackow method group and Bunnell method group. All ruptures were repaired with antimicrobic suturing material (VICRLR. PLUS), immobilized on legs by plaster cast. Meanwhile, functional training was performed. Postoperative complications and curative effect were followed up. Amer - Lindholm evaluating criteria was used to evaluate the result of the two groups. Results All patients were followed up with overall excellent rate of 96.2%, the satisfactory rate were 100.0% and 92.3% in Krackow group and Bunnell group, respectively. There were significant differences in functional recovery level at postoperotively early stage between the two groups (P 〈 0.05 ). There was no statistical difference in complication rates within 10 months(P 〉0.05). Patient's satisfaction rate in Krackow method group was higher than that in Bunnell method group, with significant statistical difference ( P = 0. 04 ). Conclusion The Krackow suturing treatments for closed rupture of Achilles tendon have good clinical results in postoperative functional recovery. Patientg satisfaction rate is higher.
