摘要
目的探讨老年肥胖患者腹部网膜及皮下脂肪组织内脂素mRNA的表达情况及其相关因素。方法根据体质量指数不同,将择期腹部手术的老年患者40例分为肥胖组20例,对照组20例。采用实时荧光定量逆转录-聚合酶链反应方法检测内脂素mRNA的表达量,酶联免疫吸附法测定血浆内脂素浓度,稳态模型评估胰岛素抵抗指数(homeostasis model of assessment for insulin resistant index,HOMA-IR),并分析内脂素与人体测量指标和代谢指标的相关性。结果①2组网膜脂肪组织内脂素mRNA的表达量明显高于皮下脂肪组织内脂素mRNA的表达量(p<0.05),肥胖组网膜脂肪组织内脂素mRNA的表达世高于对照组(P<0.05),而2组皮下脂肪组织内脂素mRNA的表达量差异无统计学意义(P>0.05);②肥胖组血浆内脂素浓度显著高于对照组(P<0.05);③合并2组数据,网膜脂肪组织内脂素mRNA的表达水平与腰臀比、空腹胰岛素、HOMA-IR呈正相关(r=0.264、0.108、0.173,p<0.05),而皮下脂肪组织未发现任何相关性因素(r=0.099,P>0.05);血浆内脂素浓度与总胆固醇呈正相关(r=0.257,P<0.05),且与网膜脂肪组织内脂素mRNA的表达水平呈正相关(r=0.267,P<0.05);④多元逐步回归分析发现,空腹胰岛素是血浆内脂素浓度和网膜脂肪组织内脂素mRNA的独立相关因素。结论内脂素mRNA主要在内脏脂肪组织中表达,可能与肥胖、胰岛素抵抗有关。
Objective To investigate the expression of visfatin mRNA in subcutaneous and omental adipose tissue and related factors in senile obesity subjects. Methods Forty senile patients of surgery were divided into obese group and normal group ( n = 20 for each) according to body mass index. The expression of visfatin mRNA in abdominal subcutaneous and omental adipose tissue was measured by real - time fluorescent quantitative reverse transcription - polymerase chain reaction ( RT - PCR) assay, plasma visfatin was measured by enzyme -linked immunosorbent assay (ELISA), and fasting blood glucose, fasting insulin (FINS), blood lipid profiles and homeostasis model of assessment for insulin resistant index( HOMA - IR) were determined. Results (~)There was no statistical difference of visfatin mRNA expression in subcutaneous adipose tissue between obese subjects and controls. Visfatin mRNA expression levels in omental adipose tissue were significantly increased in obese subjects and controls than those in subcutaneous tissue respectively( P 〈 0.05 ), visfatin mRNA expression in omental adipose tissue was significantly higher in obese group than in control group, and visfatin mRNA expression was not significantly different between obese subiects and controls. ~)Plasma visfatin levels were elevated in obesesubjects compared with controls ( P 〈 0.05 ). (~)The expression level of visfatin mRNA in all omental adipose tissue was positively correlated with waist- hip ratio ,fasting insulin, I-IOMA- insulin resistance. However the expression level of visfatin mRNA in all subcutaneous adipose was not correlated with any factors. Plasma visfatin levels were positively correlated with total cholesterol and visfatin mRNA expression levels in omental adipose tissue.
