摘要
目的:探讨老年人上消化道出血的临床特点及其治疗方法和效果。方法:2009年1月-2011年12月笔者所在医院收治的98例老年上消化道出血患者为研究对象,同时选取同期的中青年患者为研究对象,对其临床资料进行回顾性分析,分析老年人上消化道出血患者的临床表现、疼痛特点、发病原因、并发症发生情况及治疗方法和效果。结果:老年患者以呕血黑便为主要表现,腹痛为主者较少;老年人对缺血的耐受性较差,容易发生缺血性休克等并发症;老年患者的主要病因仍为消化性溃疡,但胃溃疡的发病率高于十二指肠溃疡,肝硬化、胃癌的构成比例高于中青年组;根据病因给予禁食、质子泵抑制剂、奥曲肽、三腔二囊管压迫、浓缩红细胞输注等治疗,所有患者均成功出血,观察组治愈80例,治愈率为81.63%,复发18例,复发18.37%,观察组治愈率低于对照组,复发率高于对照组(P<0.05),具有统计学意义。结论:老年上消化道出血患者具有症状不典型、耐受性差、易发生并发症、治疗效果差等特点,在临床治疗过程中需给予重视。
Objective ：To investigate the elderly, gastrointestinal bleeding, the clinical characteristics and treatment methods and effects.Methods ：Study methods of our hospital from January 2009 to December 2011, 98 cases of old age on patients with gastrointestinal bleeding, and select the same period in young and middle-aged patients for the study, the clinical data were retrospectively analyzed, in elderly of upper gastrointestinal bleeding in patients with clinical manifestations, pain characteristics, causes, complications and treatment methods and effects.Results ：In elderly patients vomiting blood, black will be the main performance, less abdominal pain ; elderly poor tolerance to ischemia, prone to ischemic complications of shock ; is still a major cause of elderly patients with peptic ulcer gastric ulcer in a high incidence of duodenal ulcer, cirrhosis of the liver, the composition ratio of gastric cancer is higher than the young group ; given fasting according to the cause, proton pump inhibitors, octreotide, three balloon catheter oppression, packed red blood cells lose Note and other treatment, all patients were bleeding, the observation group were cured, 80 cases, the cure rate was 81.63%, recurrence of 18 cases, recurrence of 18.37%, observed group the cure rate is lower than the control group, the recurrence rate was higher （P〈0.05） statistically significant.Conclusion ：Elderly patients with atypical symptoms, poor tolerance, prone to complications and poor treatment of upper gastrointestinal bleeding, tbe need to give attention to tbe clinical treatment process.
