摘要
报告了腰椎间盘突出症再手术患者 13例 ,分析其再手术原因有过于依赖 CT、定位错误、术前适应症掌握不当、术中髓核摘除不够、术后限制活动时间不足、医源性椎管和 (或 )神经根管狭窄、多节段突出处理不当、过分破坏脊柱稳定性、神经根未彻底松解及影像假象误导等。
Thirteen cases of repeated surgery of lumbar intervertebral disk hernia are reported in the paper. Through the analyses, it was found that the causes for the repeated surgery included too much dependence on CT, erroneous location, incorrect preoperative selection of the indications, inadequate intraoperative extirpation of the pulpiform nucleus, short limitation of postoperative activity, iatrogenic spinal canal or neurocanal stenosis, incorrect treatment of the multi segmental hernia, too much destroyed spinal stability, irradical lysis of the nerve root and erroneous conduction by false images. Against these causes, the preventive measures are presented in the paper.
