HLA不合异基因脐血干细胞移植治疗儿童急性髓系白血病一例并文献复习

摘要目的评价HLA不合异基因脐血干细胞移植（allo-CBT）治疗儿童急性髓系白血病（AML）的疗效。方法运用HLA4／6相合allo—CBT治疗儿童AML．M。1例，预处理方案白消安（Bu）／环磷酰胺（Cy）＋抗胸腺细胞免疫球蛋白（ATG）（阿路胞苷1．6g／d，移植前第10天和第9天；Bu15mg／次，1次／6h，移植前第8天至移植前第6天；Cy1．5g／d，移植前第5天至移植前第4天；ATG50mg，移植前第5天至移植前第2天；司莫司汀0．2g，移植前第3天）。0天回输非血缘脐血总有核细胞数12．6×10^7／kg，c啦细胞0．42×10^6／kg。移植物抗宿主病（GVHD）预防：环孢素A＋霉酚酸酯＋甲泼尼龙。结果患儿于移植后第22天白细胞植活，第32天血小板植活；第28天外周血单核苷酸序列一聚合酶链反应（STR-PCR）检测示完全嵌合体形成；移植后17个月，无慢性GVHD，无病生存。结论HLA不合allo—CBT是治疗儿童AML的有效方法之一。
