液基薄层细胞学检测在尿液、脑脊液脱落细胞学检查中的应用

摘要 目的探讨液基薄层细胞学检测(thin-layer cytologital test,TCT)在尿液、脑脊液脱落细胞学检查中的应用。方法用传统涂片和TCT制片同时对320例脱落细胞学送检标本(其中尿液228例,脑脊液92例)进行制片,比较两种方法对癌细胞的阳性检出率,并观察其病理结果。结果 TCT制片对尿液、脑脊液脱落细胞学癌细胞阳性检出率高于传统涂片,差异有统计学意义(P<0.05)。传统涂片细胞量极少,核仁不明显,着色较深,存在细胞变形现象;TCT制片细胞形态饱满,细胞核呈圆形、椭圆形,核膜清晰且光滑,染色质匀细,着色浅,可见细小的核仁。细胞质在两种涂片未见明显区别。结论 TCT制片技术对尿液、脑脊液脱落细胞学癌细胞阳性检出率及诊断准确率高,漏诊率低。 Objective To discuss the thin-layer cytologital test (TCT) method in examining urine and cerebrospinal fluid. Methods 320 cases of exfoliative cytological specimens were examined by traditional smear method and TCT method (228 cases of urine; 92 cases of cerebrospinal fluid) to compare the pathology results and detection rates. Results The positive detection rates by TCT were significantly higher than those by traditional smear for both urine and cerebrospinal fluid specimens (P 〈 0.05). In the traditional smear, cells were rare, karyosome was not clear, dyeing was strong and there were cytomorphosis. In the TCT smear, cells remained intact with circular or elliptic cell nucleus; nuclear membrane was clear, chromatin was well proportioned, and leptos karyosomes were easlily observed. There was no difference in cytoplasm between these two methods. Conclusion TCT smear can chance the positive detection rate of exfoliative cells in both urine and cerebrospinal fluid samples and reduce false negative diagnosis.
