摘要
目的:探讨多层螺旋CT(MSCT)曲面重建(CPR)成像在显示腰椎峡部裂中的临床应用价值。方法:行腰椎MSCT容积扫描,经后处理检出的腰椎峡部裂患者28例。分析其CPR成像的CT表现,并与单纯轴位平扫图像进行比较。结果:28例60处腰椎峡部裂在CPR图像上均得到清晰显示,表现为腰椎峡部不规则骨质缺损及边缘硬化。其中微小裂隙6处,不完全裂隙4处,合并骨痂、骨赘及碎骨片形成23处。单纯轴位平扫图像仅检出23例46处腰椎峡部裂,漏诊14处,漏诊率23.3%,两者对腰椎峡部裂的诊断率差异有显著性(P<0.05)。结论:MSCT、CPR成像能清晰、直观的显示腰椎峡部裂隙形态,并且能发现同时发生于多个椎体的多发性腰椎峡部裂及其周围相关继发病变的影像学征象,极大地提高了对腰椎峡部裂的诊断准确率,是检出腰椎峡部裂及其继发病变的理想成像方法。
Objective :To explore multi -slice spiral (iT(MSCT) curved planar reformation (CPR) imaging in lumbar spondylolysis in clinical application. Methods: from 2010 June up to now in our hospital of lumbar MSCT volume scanning, after detection of the spondylolysis of lumbar spine in 28 cases. Analysis of the CPR imaging of CT performance, with simple axial scan images were compared. Results:28 cases of the 60 lumbar spondylolysis in CPR images are displayed clearly, manifestations of lumbar bone defect and edge hardening rule. The tiny crack in 6, incomplete fracture 4, with bone callus, osteophyte formation and bone fragments of 23. Pure axial scan images was detected in 23 patients with 46 lumbar spondylolysis, missed 14, missed diagnosis rate was 23. 3%, both for lumbar spondylolysis diagnosis rate is significantly diflerent(P 〈0. 05 ). Conclusion : MSCT CPR are detectable in lumbar spondylolysis and its secondary lesion of the ideal imaging method.
