动态心电图判断室性早搏起源部位的分析 被引量:4

摘要 目的探讨12导联同步动态心电图(AECG)对室性早搏起源部位的定位价值。方法选择我院经12导联同步AECG检查明确诊断有频发单源室性早搏的患者90例,分析12导联AECG及该患者常规12导联心电图上室性早搏QRS波群形态的差异,对其起源部位分别进行初步定位,并与心内电生理检查结果进行对照分析。结果室性早搏起源于右室共65例,其中起源于右室流出道55例(占61.11%)、右室心底部5例(占5.56%)、右束支2例(占2.22%)、其他3例(占3.33%),起源于左室流出道20例(占22.22%)、左室心底部3例(占3.33%)、左束支2例(占2.22%)。结论根据12导联同步AECG的室性早搏QRS波群的形态可以初步判断其起源部位,指导室性早搏射频消融治疗方式的选择。 Objective To investigate the value of ambulatory electrocardiography (AECG) in identifica- tion of the site of origin of ventricular premature beats (VPBs). Methods 90 patients had been diagnosed with frequent monomorphic ventricular extrasystoles through 12-lead synchronous AECG and ECG. The location of origin of ventricular premature beats had been studied and comparatively analyzed with the results of intracardiac electro- physiology. Results There were 65 cases of VPBs identified from the right ventricle among which 55 cases from the right ventricular outflow tract (61.11%), 5 cases from the bottom of right ventricle (5.56%), 2 cases from the right bundle branch (2.22%) and 3 cases from others (3.33%). Another 25 cases were from left ventricle among which 20 cases from left ventricular outflow tract (22.22%), 3 cases from the bottom of left ventricle(3.33%) and 2 cases from the left bundle branch (2.22%). Conclusion The site of origin of VPBs can be preliminarily as- sessed according to 12-lead synchronous AECG which can help us to select proper treatment with radiofrequency ablation.
