摘要
目的回顾性探讨采用可注射性含BMP生物骨替代物经皮穿刺椎体成形术(percuaneous vertebroplasty,PVP)在治疗老年骨质疏松性胸腰椎压缩性骨折中的临床疗效与随访。方法将132例胸腰椎压缩性骨折患者随机分为两组,接受椎体成形术治疗,A组采用普通骨水泥,B组采用可注射性含BMP生物骨替代物,注入量为3~7mL,平均5mL。所有患者手术前均有明显胸背部疼痛,其中32例失去活动功能。在X线透视引导下,经双侧椎弓根入路行PVP手术,术后患者随访3~6年,平均(5.20±0.24)年。结果两组病例随访结果,A组59例疼痛改善,疼痛缓解时间为术后1~5d,平均(2.90±0.45)d,受伤节段椎体压缩修复率为(44.25±7.52)%;1例椎体注射人工骨材料溢入椎间盘,1例椎体注射人工骨材料溢入椎旁软组织,无溢入椎间孔和椎管内。B组38例疼痛改善,疼痛缓解时间为术后3~7d,平均(5.20±1.36)d,受伤节段椎体压缩修复率为(39.89±5.36)%;2例椎体注射人工骨材料溢入椎间盘,3例椎体注射人工骨材料溢入椎旁软组织,无溢入椎间孔和椎管内。结论椎体成形术为胸腰椎压缩性骨折经保守治疗无效患者提供了一种安全、微创、有效的治疗方法。
Objective To retrospectively explore the clinical curative effect and the follow-up of the injection of biological bone substitute containing BMP percuaneous vertebroplasty (PVP) in the treatment of senile osteoporosis fractures of thoracic and lumbar vertebrae in compressibility.Methods One hundred and thirty-two cases of thoracolumbar fractures compressibility patients were randomly divided into two groups,accepted vertebral angioplasty treatment,A group given common bone cement,B group given injection of BMP biological bone substitute,injection quantity was 3 to 7 mL,the average of 5 mL.All patients had obviously chest back pain before surgery,including 32 cases lose function.In X-ray guidance perspective,the bilateral pedicle into the road line PVP surgery,postoperative patients were followed up for 3 to 6 years,the average of (5.20±0.24) years.Results The results of follow-up in two groups of cases,A group of 59 cases improve pain,the pain relief time for postoperative 1 to 5 days,the average of (2.90±0.45) days,the injured segment compressed vertebral bodies repair rate of (44.25±7.52)%;In 1 case of vertebral injection into the disc with artificial bone material,and 1 case of artificial bone material vertebral body injection into the overflow of soft tissue,there without excessive enters between vertebra holes and in the spinal canal.B group of 38 cases improve pain,the pain relief time for postoperative 3 to 7 days,the average of (5.20±1.36) days,the injured segment compressed vertebral bodies repair rate of (39.89±5.36)%;Two cases of artificial bone material vertebral body injection into the spill disc,3 cases of vertebral injection into the vertebral bone material artificially overflow the soft tissue,there without excessive enters between vertebra holes and in the spinal canal.Conclusion Vertebroplasty for thoracic vertebral compression fracture conservative treatment of patients is a safe,minimally invasive,effective method of treatment.
