摘要
目的探讨维持性血液透析患者血清中性粒细胞明胶酶相关载脂蛋白(NGAL)水平与炎症指标的关系及血清NGAL水平对判断微炎症的临床运用价值。方法从2010年10月开始纳入医院血液透析患者150例,同时纳入50名健康人为对照组,收集患者及健康对照人群的人口学资料、相关的临床和生化学资料及血清NGAL、CRP、转铁蛋白饱和度(TSAT)、TNF-α、IL-6、IL-1等情况,做血清NAGL与炎症因子相关性分析。结果血液透析患者其血清NGAL水平为(445.45±50.34)ng/ml,健康对照人群为(50.02±6.45)ng/ml,差异有统计学意义(P<0.01);在微炎症组患者,血清NGAL水平、TNF-α、IL-6、IL-1、铁蛋白较无微炎症组患者均明显升高,差异均有统计学意义(P<0.05);在血液透析患者,血清NGAL水平与CRP、TNF-α、IL-6、TSAT等指标均有正相关性,差异有统计学意义(P<0.05),但与IL-1及铁蛋白差异无统计学意义。结论在血液透析患者中,血清NGAL与CRP、TSAT、TNF-α、IL-6有不同程度的正相关。
OBJECTIVE To study correlations of the level of serum neutrophil gelatinase-associated lipocalin with inflammatory factors in maintenance hemodialysis patients so as to assess the potentials of NGAL in identifying microinflammation.METHODS A total of 150 maintenance hemodialysis patients and 50 healthy people as the control group were enrolled in this study,the demographic data,clinical and biochemical data of those subjects were collected,the serum concentration of NGAL,CRP,transferrin saturation,TNF-α,IL-6,and IL-1 were measured.The correlation between serum NGAL and inflammatory factors was analyzed.RESULTS The concentration of serum NGAL was significantly higher in hemodialysis patients（445.45±50.34） ng/ml than in the control group（50.02±6.45） ng/ml,the difference was statistically significant（P〈0.01）;in the patients with microinflammation,the levels of serum NGAL,TNF-α,IL-6,IL-1,and ferritin were significantly elevated as compared with those in the patients without microinflammation,the differences were statistically significant（P〈0.05）;among the patients with microinflammation,serum NGAL was positively correlated to CRP,TNF-α,IL-6,and TSAT,the difference was statistically significant（P〈0.05）,but there was no positive correlation with IL-1 and ferritin.CONCLUSION Among the hemodialysis patients,the serum level of NGAL is positively correlated to CRP,TSAT%,TNF-α,and IL-6 in varying degrees.
