摘要
目的探讨体液循环中血清补体因子H的表达与糖脂代谢和胰岛素抵抗之间的相关性。方法将60例2型糖尿病患者按体质指数(BMI)分为肥胖组(BMI≥25 kg/m2)32例与非肥胖组(BMI﹤25 kg/m2)28例;30例同期健康体检正常者为对照组。采用酶联免疫吸附试验法检测血清补体因子H,放射免疫法测定胰岛素,全自动生化分析检测空腹血糖、血脂,全自动糖化分析仪测糖化血红蛋白(HbA1c);同时测量身高、体质量、血压、腰围、臀围、收缩压、舒张压,计算BMI和腰臀比(WHR),并计算胰岛素抵抗指数(HOMA-IR)。结果 3组受检者血清补体因子H比较,差异有统计学意义(P<0.05)。补体因子H水平与BMI、空腹血糖、HOMA-IR、HbA1c、空腹胰岛素、三酰甘油水平均呈正相关(P<0.01)。结论 2型糖尿病血清中补体因子H水平的升高,尤其是肥胖的2型糖尿病患者更明显,血清补体因子H可能参与了2型糖尿病肥胖患者的胰岛素抵抗的形成。2型糖尿病患者血清补体因子H水平均与胰岛素抵抗有关,表明在一定程度上,补体因子H水平可成为预测肥胖者发展为糖耐量异常的因子。
Objective To investigate the correlation between serum compliment factor H and glucolipid metabolism and insulin resistance in patients with type 2 diabetes.Methods 60 patients with type 2 diabetes were divided into obese group(BMI≥25 kg/m2,32 cases) and non-obese group(BMI25 kg/m2,28 cases) according to BMI.Meanwhile,30 healthy people were involved as control group.Serum complement factor H was determined by ELISA approach and the level of insulin was tested by radioimmunoassay.Fasting blood glucose and fasting blood lipid were tested by automatic biochemical analyzer and HbA1c was measured by automatic saccharification analyzer.Height,body weight,blood pressure,waist circumference,hip circumference,systolic pressure and diastolic pressure were tested at the same time to calculate BMI and WHR as well as HOMA-IR.Results The levels of serum complement factor H showed statistically significant differences between obese group,non-obese group and control group(P0.05).Correlation analysis showed that serum complement factor H was positively correlated with BMI,HOMA-IR,fasting blood glucose,HbA1c,fasting insulin and triglyceridemia(P0.05).Conclusion The levels of serum complement factor H were increased significantly in obese group compared with those in control group and non-obese group,and the serum complement factor H may be involved in the occurrence of insulin resistance in T2DM patients.Correlation analysis showed that serum complement factor H was positively correlated with BMI and HOMA-IR,indicating that it may predict the development from obesity to impaired glucose tolerance(IGT).
