摘要
目的探讨CT引导立体定向置管引流术治疗高血压脑出血的临床疗效。方法回顾性分析我科2007年1月～2011年12月收治的120例病人的临床资料。其中出血位于基底节区82例,丘脑38例。出血同时破入脑室20例。出血量15～45ml。临床分级评定:I～III级。全部病例均采用CT引导立体定向置管引流术治疗,出血破入脑室者同时行侧脑室引流术。结果死亡8例,占6.7%。术后血肿增大11例,均在24h内,6例行开颅血肿清除,5例继续引流治愈。85例得到1～2个月复查或随访,按脑出血日常生活能力评分标准(ADL)评定:I级49例,II级20例,III级14例,Ⅳ级2例。结论 CT引导立体定向置管引流术治疗高血压脑出血效果理想。
Objective To study the clinical effect of CT-guided Stereotactic drainage in the treatment of hypertensive cerebral hemorrhage.Methods Retrospective analysis the clinical data of 120 cases treated by our department in January 2007~ December 2011.Of them,there were 82 cases of Hemorrhage in basal ganglia,38 cases of thalamus.20 cases of breaking into ventricle hemorrhage at the same time.Bleeding volume was ranged from15ml to 45ml.Clinical grading evaluation： I ~ III.All cases were treated by CT-guided stereotactic drainage treatment,the cases with hemorrhage breaking into ventricle treated by lateral ventricle drainage at the same time.Results Of the 120 cases,8 cases died,accounting for 6.7%.Postoperative hematoma enlargement in 11 cases,in the 24h,there were 6 cases treatded by craniotomy hematoma removal,5 cases healed by continuous drainage.according to cerebral hemorrhage ADL score standard（ADL）： evaluation of class I in 49 cases,class II in 20 cases,class III in 14 cases,class IV in 2 cases.Conclusion The effect of the CT-guided Stereotactic drainage in the treatment of hypertensive cerebral hemorrhage is satisfactory.
