摘要
目的 报道逆行浅静脉 -皮神经营养血管皮瓣的临床应用效果 ,探讨浅静脉 -皮神经营养血管皮瓣的血供。 方法 用逆行头静脉 -前臂外侧皮神经营养血管皮瓣修复腕部及手背软组织缺损 6例 ,逆行小隐静脉 -腓肠神经营养血管皮瓣修复踝周及足后跟软组织缺损 5例。 结果 11例中 10例皮瓣全部成活 ,1例远端小部分坏死 ,随访 4～ 2 6个月 ,皮瓣质地优良 ,外观及功能满意。 结论 逆行浅静脉 -皮神经营养血管皮瓣血供可靠 ,切取容易 ,不牺牲主干动脉。逆行头静脉 -前臂外侧皮神经营养血管皮瓣可满意地修复腕及手背部软组织缺损 ,逆行小隐静脉 -腓肠神经营养血管皮瓣是修复踝周及足后跟软组织缺损的理想供区。
Objective To report the results of distally based veno neurocutaneous vascular island flaps in the extremities in clinical application and to exploring the blood supply of veno neurocutaneous vascular flap Methods Six cases with soft tissue defects arround the wrist and the dorsum of the hand were treated with distally based cephalic antebrachial veno neuro vascular island flaps,another 5 cases with soft tissue arround the lateral malleolus and the heel were treated with distally based lesser saphenous sural veno neuro vascular island flaps Results The flaps totally survived in 10 cases,the other was small distal necrosis The color and texture of the flaps were extremely good,their appearance and function were satisfactory after 5～26 months follow up Conclusion The distally based veno neurocutaneous vascular island flaps have the reliable blood supply and the character of avoidance of sacrificing the major arteries of the extremity and are easy to elevate,The distally based cephalic antebrachial veno neuro vascular island flap is satisfactory in repairing the soft tissue deffects arround the wrist and the dorsum of the hand,the distally based lesser saphenous sural veno neuro vascular island flap is an ideal flap for repairing the soft tissue deffect around the malleolus and the heel
