摘要
目的探讨胎心监护在胎盘早剥早期诊断中的应用价值。方法胎盘早剥患者52例,其中早剥面积<1/3有30例(轻型组),早剥面积≥1/3有22例(重型组),分析2组的胎心监护、超声检查和妊娠结局,并进行比较。结果 (1)胎盘早剥在临产前诊断27例,其中轻型组10例,重型组17例;产程中诊断17例,均为轻型组;产后诊断8例,轻型组3例,重型组5例。B超和胎心监护异常共同诊断22例,其中轻型组5例,重型组17例。(2)52例者中,胎心监护异常43例,其中胎心率(FHR)心动过速14例,FHR心动过缓10例,FHR静止型18例,FHR跳跃型6例,中重度变异减速2例,晚期减速14例。(3)重型组中,产后出血9例,子宫卒中5例,行B-lynch缝合止血6例,输血治疗11例;轻型组未发生上述情况。无切除子宫,无产妇死亡。结论胎心监护对胎盘早剥的早期诊断具有重要的临床应用价值。
Objective To investigate application the fetal heart rate monitoring in the early diagnosis of placental abruption.Methods Retrospective analysis 52 cases of delivery in patients with placental abruption,fetal heart rate monitoring results since August 2006 to October 2011 at our hospital.According to placental abruption area less than one-third of the 30 cases（the mild group）,placental abruption area greater than or equal two-third of the 22 cases（the severe group）,compared two groups of fetal heart rate monitoring,ultrasound and pregnancy result.Results The diagnosis of placental abruption was 27 cases before labor,the mild group was 10 cases,the severe group was 17 cases;17 cases labor processing diagnosis all were the mild group;8 cases postpartum diagnosis,the mild group was 3 cases,the severe group was 5 cases.B-ultrasound and fetal heart rate monitoring abnormalities same diagnosis in 22 cases,5 cases of the mild group and 17 cases of the severe group;For surgical treatment of abnormal fetal heart rate monitoring confirmed 21 cases all were the mild group;the mild group 1 case diagnosis of placental abruption case of B-ultrasound cesarean section.Conclusion The performance of placental abruption sharp drop in blood supply,leading to fetal distress,fetal heart rate changed,direct expression of abnormal fetal heart rate monitoring,therefore,placental abruption,fetal heart rate monitoring for early diagnosis has very important clinical value.
