摘要 目的:探讨延期手术治疗Rued-AllgowerⅢ型Pilon骨折的临床疗效。方法:自2007年1月至2010年10月应用延期手术治疗Rued-AllgowerⅢ型Pilon骨折20例,其中男性15例,女性5例,年龄25-60岁,平均40.5岁,开放性骨折6例,闭合性骨折14例,合并腓骨骨折16例,均给予延期切开复位内固定治疗。结果:随访时间为0.5-4.0年,平均3年,全部获得骨性愈合,无骨不连存在,骨折愈合时间2-6个月,平均3个月,按照Mazur等制定的踝关节评分系统,其中优7例,良10例,可2例,差1例,优良率95%。无一例出现骨外露、深部感染、骨折移位畸形等并发症。结论:延期手术治疗Rued-AllgowerⅢ型Pilon骨折能有效减少并发症和良好的恢复踝关节功能, Objective: To investigate the clinical effects of delayed surgery for Rued-AllgowerⅢ pilon fracture. Method: Analyzed the clinical data of 20 patients who had the delayed surgery for Rued-AllgowerⅢ pilon fracture from Jan.2007 to Oct.2010.There were 15 males and 5 females with the average age of 40.5 years and the age ranging from 25 to 60 years.6 patients had open fracture,14 patients had closed fracture and 16 patients had concomitant fibula fractures and they were all given delayed internal fixation. Result: The average following-up period was 3 years ranging from 0.5 to 4.0 years and all the patients had healed fracture without nonunion.The average period of healing was 3 months and ranged from 2 months to 6 months.According to the ankle scoring system formulating by Mazur,there were 7 patients got very good,10 patients got good,2 patients got not bad and 1 patient got bad.The excellent rate was 95%.No bone exposed,deep infection,fracture displacement and other complications were found. Conclusion: Delayed surgery for Rued-AllgowerⅢ pilon fracture can reduce complications effectively and recover the function of ankle joint.
