摘要
目的评价多层螺旋CT血管成像(MSCTA)在外周假性动脉瘤中的临床应用价值。资料与方法搜集2008年9月至2011年7月临床拟诊假性动脉瘤患者19例并行MSCTA检查,对比剂总量80~90 ml,注射流率3.0~3.5 ml/s,扫描延迟时间25~35 s,将获得的容积数据传至图像工作站进行多种后处理,重组技术包括多平面重组(MPR)、最大密度投影(MIP)和容积再现(VR)。结果 19例外周假性动脉瘤的MSCTA 4例为外科手术证实,15例同期行数字减影血管造影(DSA)证实,其中位于股总动脉6例,股浅动脉8例,股深动脉2例,肱动脉2例,胫后动脉1例,其中17例破口清晰显示,15例瘤腔周边见不规则低密度血栓或高密度血块影,4例仅显示假性动脉瘤瘤体、载瘤动脉及其破口。结论 MSCTA是一项简便、无创而可靠的影像检查技术,可以很好地显示假性动脉瘤的特征性影像,有重要临床应用价值,可作为外周假性动脉瘤的首选检查方法。
Objective To evaluate the clinical value of multislice spiral CT angiography(MSCTA) in the diagnosis of peripheral pseudoaneurysms(PPA).Materials and MethodsMSCTA was performed in 19 cases with clinic suspected of peripheral pseudoaneurysms.The CTA data of all patients were reconstructed with multiple planar reformation(MPR),maximum intensity projection(MIP) and volume rendering(VR).ResultsMSCTA was successfully performed in all 19 cases with peripheral pseudoaneurysms,the diagnostic accuracy of MSCTA was 100%(19/19).The peripheral pseudoaneurysms located at common femoralartery in 6 cases,8 cases at superficial femoral artery,2 cases at deep femoral artery,2 cases at humeral artery and 1 case at posterior tibial artery,the lacuna of PPA could be displayed clearly in 17 cases,hypodensity thrombus and high density clots around the lumen could be exhibited in 15 cases,the pseudoaneurysms,the adjacent parent artery and the lacuna of PPA only could be showed in 4.ConclusionMSCTA is a noninvasive and reliable technique which can demonstrate clearly the characteristic images of PPA and has important clinical value,MSCTA can be used as a primary method in diagnosis of peripheral pseudoaneurysms(PPA).
