应用斑点追踪成像技术评价心脏再同步化治疗后局部心肌功能变化

摘要 目的:研究慢性心力衰竭患者心脏再同步化治疗(CRT)术后局部心肌纵向、径向、圆周方向心肌收缩运动及旋转角度随时间的动态变化规律,并评估CRT术后左心室各方向收缩同步性。方法:应用斑点追踪成像技术分析15例接受CRT治疗的慢性心力衰竭患者术前、术后1周、术后3月局部心肌纵向应变、径向应变、圆周应变及旋转角度,观察纵向应变、径向应变、圆周应变及旋转角度达峰时间,获得不同方向达峰时间标准差。结果:CRT术后1周心肌纵向应变达峰时间标准差减小(P<0.05),径向、圆周、应变及旋转角度达峰时间标准差较术前无明显差异;术后3月径向应变、圆周应变、旋转角度达峰时间标准差均减小(P<0.05)。术后1周心肌纵向应变及圆周应变较术前增大,径向应变及旋转角度较术前无明显差异,术后3月心肌纵向、径向、圆周应变及旋转角度均较术前明显增高(P<0.05)。结论:应用斑点追踪成像技术能评价CRT术后心肌局部功能,CRT治疗有效者术后心肌局部功能恢复在心肌不同收缩方向存在动态变化过程。 Objective: To find the dynamic regional myocardial function and its systolic synchronization in patients of chronic heart failure who were treated with cardiac resynchronization therapy(CRT),The longitudinal strain radial strain,circumferential strain and rotation were observed.Methods: Fifteen patients suffering from heart failure,who received CRT,were select.Speckle tracking imaging in addition to standard echocardiography were performed prior to CRT,one week and 3 months after CRT.We observed the longitudinal strain,radial strain,circumferential strain and rotation,as well as time to peak strain and peak rotation.The standard deviation of time to each peak systolic strain and rotation(Ts-sl-SD,Ts-Sr-SD,Ts-sc-SD,Ts-srot-SD)were calculated.Results: One week after CRT,the Ts-sl-SD shortened(P0.05),but there were no significant difference in Ts-Sr-SD,Ts-sc-SD,and Ts-srot-SD(P0.05).Three months after CRT,all of them were shortened(P0.05).The longitudinal and circumferential strain increased one week after CRT compared with those prior to CRT(P0.05).Three months after CRT,not only longitudinal and circumferential strain but radial strain and rotation increased(P0.05).Conclusions: Regional myocardial function can be valued by speckle tracking imaging.There is dynamic change in regional myocardial function of different direction of cardiac contraction in patients who response to CRT.
