摘要
目的:探讨大鼠自体原位肝移植改良模型的建立技巧。方法:回顾性分析同一操作者学习和建立216例大鼠自体肝移植模型的过程,总结手术技巧,手术改良方法。按学习时间先后分为预实验组(Y组)30例,模型改良组(G组)36例,模型稳定组(W组)150例,观察手术成功率、手术时间、无肝期时间、手术后8h存活率及手术后并发症;W组随机选取20例大鼠8h后处死留取肺肾组织标本,光镜观察肺肾损伤发生情况。结果:Y组手术成功率为73.3%(22/30),8h存活率20%(6/30),G组手术成功率为83.3%(30/36),8h存活率63.9%(23/36),W组手术成功率为98%(147/150),8h存活率83.3%(128/150),死亡原因主要为出血、门静脉狭窄和低温等等。20例W组肺损伤发生率35%(7/20),肾损伤发生率45%(9/20)。结论:熟练的手术技巧、缩短无肝期时间、减少损伤和出血,注意保温是模型成功的关键;改良模型能满足麻醉基础研究中有关器官保护研究的需要。
Objective To explore the technique of establishing the orthotopic autologous liver transplantation model of rats. Methods The process of studying and establishing 216 orthotopic autologous liver transplantation models of rats of an operator was analyzed retrospectively, the technique and improvement of the operation were summarize. Aceording to the time of study, the rats were divided into 3 groups: pre-experimental group (Group Y, n=30), improved model group (Group G, n=36), and stable model group (Group W, n=150). The suecess rate, operating time, anhepatic phase, livability (8h after operation), and postoperative complications were observed. 20 rats in Group W were randomly chosen and sacrificed 8 h later; the lung and kidney tissues were taken, and the injury was observed. Results The success rate of the operation of Group Y, G, and W were 73.3% (23/36), 83.3% (30/36), and 98% (147/150), respectively; and the livability (8 h after operation) of the three groups were 20% (6/30), 63.9% (23/36), and 83.3% (7/20), respectively. The main causes of death were bleeding, stenosis of portal vein, hypothermia and so on. The incidence of lung injury and renal injury in Group W (20 picked rats) were 35% (7/20) and 45% (9/20). Conclusion Skilled surgical techniques, less anhepatic phase, limited injury or bleeding, and keeping warm were the key to establishing the model ; the improved model can meet the need of the research of organ protection during the basic research of anesthesia.
