摘要
目的:弄清供应手内在肌的血管,区分手内在肌的主要血管蒂,为这些肌肉作为肌瓣或肌皮瓣提供解剖学基础。探讨利用手内在肌作为肌瓣的临床应用。方法:利用氧化铅明胶灌注技术,通过股动脉对20具新鲜尸体进行灌注。解剖手内在肌,仔细观察和计数每一块手内在肌的血管蒂的数目、类型、血管大小及分布区域。结果:手内在肌的血管主要来自手掌侧动脉的分支,这些血管的直径大小范围为0.6~1.4mm。骨间背动脉的血管沿着骨间背侧肌的尺侧和桡侧走行。骨间背侧肌肌瓣成功地在5例临床病人中应用。结论:手内在肌的解剖学研究为设计皮瓣及原位皮瓣转移打下基础。基于本实验研究表明,手内在肌可作为近侧或远侧肌瓣应用。
Objective: To detect the vascular supply of the intrinsic muscles of the hand and the dominant arterial pedicle of each muscle,which were used potentially as muscle or musculocutaneous flap.Methods: The radio-opaque lead oxide gelatin injection was injected into the femoral artery of 20 fresh cadavers.The intrinsic muscles of the hand were meticulously dissected with their vascular pedicles.The number,type,diameter of vascular pedicles of muscles,and their distribution were analyzed.Results: The intrinsic muscles were supplied by branches of the metacarpal arteries.The diameters of the dominant branches ranged from 0.6 to 1.4 mm.The vascular territories of the dorsal metacarpal arteries extend along the ulnar and radial sides of the dorsal interosseous muscles.The dorsal interosseous muscle flaps have been used in 5 clinical cases.Conclusion: This anatomic study provides further information to help design flaps from the hand for local tissue transfer.Based on this study,interosseous muscle of the hand may be harvested as a proximal or distal muscle flap.
