摘要
J波指心电图QRS波结束和ST段起始的结合点(J点)抬高,振幅≥0.1 mV,时程≥20 ms,向上圆顶样或驼峰样偏离基线的波。近年来的研究中将与心电图J波有关,具备相同离子流机制,具有室性心动过速-心室颤动-心脏性猝死的倾向,合并或不合并器质性心脏病的临床综合征统称为J波综合征,认为其某些类型成为恶性室性心律失常心脏性猝死高危预警的新指标。现就近年来J波综合征在离子流和细胞电生理机制、遗传学基础、分类分型与临床意义以及治疗方面的重要进展进行综述。
J wave refers to a dome or hump wave on electrocardiogram which drifts off the baseline and is formed from an elevation of the QRS-ST junction(J point),amplitude≥0.1mV,period≥0.2ms.J wave syndromes was defined in researches in the past few years,referring to a series of collective clinical syndromes characterized by J wave on electrocardiogram,with the same ionic mechanism,the tendency of ventricular tachycardia-ventricular fibrillation-sudden cardiac death and combining with or without organic heart disease.Certain types of J wave syndromes have become new high-risk warning index of malignant ventricular arrhythmia and sudden cardiac death.So here is to make a review on the important research progresses of J wave syndromes in mechanism of ion flow and cell electrophysiology,genetic basis,clinical significance of classification and syndrome types and treatment in recent years.
