摘要
目的观察药物性急性间质性肾炎(D-AIN)肾组织浸润炎性细胞的特征及意义。方法将重复肾活检的D-AIN患者12例分为肾功能完全恢复组(A组)和未完全恢复组(B组),每组6例。应用免疫组化法观察T淋巴细胞[CD3(+)]、单核巨噬细胞[CD68(+)]及B淋巴细胞[CD20(+)]的浸润、肾间质微血管密度以及细胞外基质的沉积。结果①2组性别、基础肾功能和病理损害程度等无明显差异;B组第2次肾活检时出现肾间质纤维化。②第1次肾活检时,2组均表现为显著的炎症细胞浸润。A组第2次肾活检时各种炎症细胞均明显减少,而B组[CD68(+)]细胞未受抑制,成为主要的浸润细胞,伴有胶原沉积明显增加。结论持续单核巨噬细胞浸润可能是D-AIN患者病变迁延的重要机制。
Objective To observe the characteristics of the inflammatory cells in drug-induced acute interstitial nephritis (D-AIN). Methods Twelve cases of D-AIN subject to renal re-biopsy were divided into recovered and non-recovered groups. Immunohistochernical SP method was applied to detect the infiltration of T lymphocytes [CD 3 ( + )], monocytes/macrophages[CD 68 ( + )] and B lymphocytes [CD 20 ( + )], the density of peritubular capillaries [PTC, CD 34( + )], and the expression of a-SMA, collagen III and IV in the renal biopsy specimens. Results The two groups presented comparative clinicopathological features, as well as the degrees of inflammatory cells infiltration. After treatment for 1-4 months, patients in the recovered group showed significant decrease of inflammatory infiltration in the re-biopsy kidneys, while those in the non-recovered group presented persistent monocytes/macrophages infiltration and significant interstitial fibro- sis. Conclusions Monocytes/macrophages might play very important roles in the transition from acute in- flammation into chronic fibrosis in D-AIN.
