摘要
目的探讨实时三维超声在胎儿畸形的筛查和诊断中的临床应用价值。方法采用实时三维超声对3 758例孕14~41周胎儿进行二维及三维超声检查,并与出生后及引产结果相比较。结果在3 758例孕14~41周的胎儿中,经引产或出生后证实的各种畸形266例,二维超声产前检出256例,漏诊10例,二维超声诊断符合率96.2%(255/266);二维超声诊断胎儿畸形的敏感度96.2%,特异度100%,准确度99.7%,阳性预测值100%,阴性预测值99.7%,实时三维超声产前检出263例,漏诊3例,诊断符合率为98.9%(263/266),实时三维超声诊断胎儿畸形的敏感度98.9%,特异度100%,准确度99.9%,阳性预测值100%,阴性预测值99.9%。结论实时三维超声对胎儿畸形的筛查和诊断具有较高敏感度和特异度,在胎儿畸形的产前诊断方面具有较高的应用价值。
Objective To explore the value of clinical application of real-time three dimensional ultrasonography on screening and diagnosis of fetal malformations.Methods The real-time three dimensional ultrasound performance in 3 758 cases of fetuses ranging in gestational age between 14-41 weeks were examined with two dimensional ultrasound and three dimensional ultrasound,and compared by artificial labor and postnatal follow-up visit result.Results 266 cases of fetal malformations were confirmed after abortion or delivery in 3 758 cases of fetuses ranging in gestational age between 14-41 weeks,256 cases were diagnosed correctly by two dimensional ultrasound,10 cases were missed,the sensitivity was 96.2%,specificity was 100%,accuracy was 99.7%,positive predictive value was 100%,negative predictive value was 99.7%;263 cases were diagnosed correctly by real-time three dimensional ultrasound,3 cases were missed.The diagnosis rate of fetal malformation of real-time three dimensional ultrasound was 98.9%,sensitivity was 98.9%,specificity was 100%,accuracy was 99.9%,positive predictive value was 100%,negative predictive value was 99.9%.Conclusion The sensitivity and specificity of real-time three dimensional ultrasound in screening and diagnosis of fetal malformations are high,it has good practical value of prenatal diagnosis in fetal congenital abnormalities.
