2种升压药对剖宫产新生儿血气影响的Meta分析

Effects of 2 Kinds of Hypertensors on Blood Gas of Caesarean Delivery Newborns: A Meta-analysis

摘要 目的:利用Meta分析方法评价苯肾上腺素和麻黄素对剖宫产新生儿血气的影响。方法:查阅中国期刊全文数据库(CNKI)、PubMed、万方数据库、维普中文科技期刊全文数据库(VIP)、中国生物医学文献数据库(CBM)2000-2011年3月公开发表的随机对照临床试验文献,按照纳入与排除标准选择文献、评价质量和提取数据,采用RevMan4.2软件进行Meta分析。结果:纳入10篇文献,2组新生儿脐动脉血的pH值[WMD=0.05,95%CI(0.04,0.06),P<0.00001]和氧分压(PO2)[WMD=0.52,95%CI(0.08,0.96),P=0.02]以及二氧化碳分压(PCO2)[WMD=-4.35,95%CI(-5.00,-3.69),P<0.00001]差异均有统计学意义;2组新生儿脐动脉血的剩余碱(BE)[WMD=1.38,95%C(I-0.15,2.92),P=0.08]差异无统计学意义。结论:苯肾上腺素有利于增加胎儿氧供,降低缺氧性酸中毒,值得临床推广应用。 OBJECTIVE: To evaluate the effects of phenylephrine and ephedrine on blood gas of caesarean delivery newborns by Meta-analysis. METHODS: We searched literatures published during 2000-Mar. 2011 from CNKI, PubMed, Wanfang database, VIP and CBM. The methodological qualities of the included studies were evaluated and the data was extracted according to inclusion and exclusion criteria. Meta-analyses were performed using RevMan 4.2 software. RESULTS: A total of 10 studies were included. The results of Meta-analyses indicated that: the pH value [WMD=0.05,95%CI(0.04,0.06),P0.000 01], PO2 [WMD=0.52,95%CI(0.08,0.96),P=0.02] and PCO2 [WMD=-4.35,95%CI(-5.00,-3.69),P0.000 01] in arteria umbillicalis of the two groups had significant difference. However, there was no significant difference in BE value [WMD=1.38,95%CI(-0.15,2.92),P=0.08] between the two groups. CONCLUSION: Phenylephrine contributes to the oxygen supply and decreases the incidence of acidosis in infants. It is worthy of being popularized in the clinic.
