摘要
目的 分析合并心脏恶液质的风湿性心胖瓣膜病外科治疗的结果。方法 近８年间对３４例风湿性心脏瓣膜病伴心脏恶液质病人施行二尖瓣瓣膜置换术和／或主动脉瓣置换术，其中２９例同时施行三尖瓣成形术。心功能Ⅲ级１５例、Ⅳ级１９例。结果 早期死亡４例（１１．８％），死亡原因主要为多脏器功能衰竭及心衰，晚期死亡１例。结论 心脏恶液质 人术后易发生各种并发症。围术期加强改善营养及心功能。
Objective:To analyse the result of surgical treatment of the rheumatic valvular heart disease with cardiac cachexia.Methods:During the last 8 years,mitral and /or aortic valve replacement were performed in 34 patients with rheumatic valvular heart disease combined with cardiac cachexia,29 of them were treated simultaneously by tricuspid annuloplasty.Heart function was class Ⅲ in 15 patients and class Ⅳ in 19 patients.Results:There were 4 early operative deaths with a mortality rate of 11 8%.The causes of death included the function failure of several organs and heart function failure.1 patient died of brain breeding in 27 months after operation.Conclusion:The patients with cardiac cachexia are susceptible to the various complications after operation.Their nutrition and heart function must be improved in perioperative period.The myocardial protection and tricuspid treatment should be strengthened in operation.Treatment of postoperative complications is critical for the surgical results.
