摘要
目的了解中药膳食对严重烫伤大鼠肠源性感染防治效果,探索临床防治烧伤后肠源性感染的新方法。方法健康Wistar大鼠100只,随机分为药膳组、肉汤组、常规组(n=30)和对照组(n=10);药膳、肉汤、常规组于伤后第1、3、7 d各取10只,无菌条件下取材检测肠粘膜病理形态学、血浆内毒素含量、细菌移位率、盲肠膜菌群、肠黏液中分泌型免疫球蛋白A(sIgA)含量。结果与对照组比较,伤后药膳、肉汤、常规组细菌移位率、内毒素水平升高、酵母菌及大肠杆菌数量明显增多,肠道slgA水平由(59.31±8.66)μg/mL降至(38.12±4.49)μg/mL、双歧杆菌数量由(7.6±0.6)log CFU/g降至(6.3±0.5)log CFU/g(P<0.05);与肉汤、常规组比较,药膳组细菌移位率、内毒素水平降低、酵母菌及大肠杆菌明显减少,肠道slgA水平升高至(53.06±5.23)μg/mL,双歧杆菌数量明显增多(P<0.05)。结论严重烫伤破坏了肠道粘膜屏障功能,引起细菌、内毒素移位,应用中药膳食干预,能有效预防肠源性感染。
Objective To examine therapeutic effect of Chinese medicinal diet on gut derived infection after severe burn in rats.Methods Totally 100 healthy Wistar rats were randomly divided into four groups:the burnal rats with medicinal diet gavage group(medicinal diet group,n=30),the burned rats with bouillon gavage group(bouillon group,n=30),the burned rats with conventional feeding group(conventional group,n=30),and the normal control group(normal group,n=10).At 1,3,7 day after the burn,10 rats in medicinal,bouillon,and conventional group were killed under asepsis condition to detect the pathomorphological change of intestinal mucosa,the intestinal bacteria translocation rate,the endotoxin level,cecum mucosal microflora,and the level of secretory immunoglobulin A(sIgA).Results There were pathomorphological changes in intestinal mucosa of the rats in medicinal diet,bouillon,and conventional group after the burn.The rats in medicinal group recoverd 7 days after the burn,but the rats in bouillon and conventional group didn't completely recoverd.Compared with the normal group,the intestinal bacterial translocation rate,the endotoxin level,the yeast and E.Coli of the rats in medicinal,bouillon,and conventional groups were increased significantly while the level of sIgA and the bifidobacteria were decreased after the burn(P〈0.05 or P〈0.01).Compared with the medicinal group,the intestinal bacterial translocation rate,the endotoxin level,the yeast and E.Coli of the ras in bouillon and conventional group were increased significantly while the level of sIgA and the bifidobacteria was decreased after the burn(P〈0.05 or P〈0.01).Conclusion Application of traditional Chinese medicinal diet could treat gut derived infection after burn in rats.
