摘要
目的:分析中原地区不同HPV基因亚型与宫颈癌前病变程度的关系。方法:采用核酸分子快速杂交基因分型技术检测240例宫颈感染HPV患者的基因亚型,根据细胞与病理学诊断(阴道镜活检)将研究对象分为3组:①宫颈癌与高度上皮内病变组(HSILs)80例;②低度上皮内病变组(LSILs)75例;③正常或炎症组85例。根据HPV-DNA检测结果,分析中原地区不同HPV基因亚型与宫颈癌前病变程度的关系。结果:3组患者中HPV-H阳性率分别为93.6%、62.8%、26.8%,两种以上HPV-H基因型感染者分别占37.2%、16.8%和12.6%,HSIL组明显高于其他两组,差异有统计学意义(P<0.01);感染率从高到低依次为HPV-16、52、58、33、18、31、11(低危型)、56、53、68、59、39、CP8304、66、6(低危型)、45、35、51和低危型中44、42、43。结论:HPV-H与宫颈癌变密切相关,该组宫颈病变患者感染HPV型别以16、52、58、33较为多见。
Objective:To analyze the relationship between different human papillomavirus(HPV) genetic subtypes and the degree of cervical precancerous lesion in central plains.Methods:DNA flow-through hybridization genotyping technique was applied to detect HPV genetic subtypes of 240 patients with cervical HPV infection,then the study objects were divided into three groups according to cytological diagnosis and pathological diagnosis(colposcopic biopsy): 80 cases in cervical cancer and high-grade squamous intraepithelial lesion(HSIL) group,75 cases in low-grade squamous intraepithelial lesion(LSIL) group,and 85 cases in normal or inflammation group.The relationship between different HPV genetic subtypes and the degree of cervical precancerous lesion in central plains was analyzed according to the detection results of HPV-DNA.Results:The positive rates of HPV-H in the three groups were 93.6%,62.8%,and 26.8%,respectively.The proportions of the cases with more than two kinds of HPV-H genetic subtypes infection in the three groups were 37.2%,16.8%,and 12.6%,respectively,the proportions in HSIL group were significantly higher than those in the other two groups(P0.01).The infection rates listed from high to low were HPV-16,52,58,33,18,31,11(low risk type),56,53,68,59,39,CP8304,66,6(low risk type),45,35,51,and 44,42,43(low risk type).Conclusion:HPV-H is closely associated with cervical cancer.HPV 16,52,58,and 33 are the common HPV types among the cases with cervical lesions in the study.
