摘要
目的:了解居民对健康档案的认知和态度,探索建立完整健康档案的有效方法。方法:采用自行设计的调查问卷,应用随机分层抽样方法抽取4个社区18岁以上居民进行健康档案认知和态度问卷调查。结果:调查775人,显示调查人群健康档案知识掌握一般,平均得分(4.65±2.54)分,不同人群的健康档案知识得分有差异,以50～70岁、文化程度高、公务员和学生职业的人群掌握更多健康档案知识。38.07%认为建立健康档案无意义或意义不大,仅13.16%的人认为应主动建档。社区居民知晓健康档案信息的来源主要是社区卫生工作人员。不愿建档的原因主要是害怕泄露个人信息和害怕上当受骗。结论:社区居民对健康档案的认知情况一般,需要加强对中青年、低学历和农民等人群的健康档案知识宣传;加强建档过程中的沟通和健康档案的管理,与居民建立良好互信关系;建立健康档案信息平台,动态使用和管理健康档案,体现建立健康档案的意义。
Objective ：To understand the community residents of the cognition and attitude of health archives, explore the establishment of an effective way to complete health archives. Methods： Using self designed questionnaire,application of stratified sampling method to select 4 residents above 18 years of age are health archives awareness and attitudes questionnaire. Results： Survey 775 people, the results showed that health archives knowledge in general, the average score of（4.65 ＋ 2.54） points, different groups of health ttrchives knowledge score difference, 50 to 70 years old, high education level, career civil servants and students have more knowledge of health records. 38.07% people believed that the establishment of health records meaningless or meaningful, only 13.16% people think that filing should take the initiative. Community residents know health record information sources mainly community health work ers,The main reason is unwilling to archiving fear disclosure of personal information and fear of being cheated. Conclusion：The community inhabitants are ordinary to the healthy file cognition situation, need to strengthen the middle aged, farmers and other groups with low education propaganda of health re cords, Strengthen the process of communication and archiving health records management, Establish a good relationship of mutual trust with the residents, es tablishment healthy file information platform, dynamic use, and manage health records, reflect the significance of the establishment of health records.
