摘要
目的:分析男性青年阻塞性睡眠呼吸暂停低通气综合征OSAHS患者整夜多导睡眠监测PSG的特点。方法:对我院睡眠医学中心诊断的231例男性青年OSAHS患者的整夜PSG资料进行回顾性分析,对患者的一般情况、呼吸暂停和低通气情况、氧减饱和情况、睡眠结构以及各指标间可能的相关关系进行统计分析。结果:该组OSAHS患者超重率93.07%,肥胖率为75.76%。中位呼吸暂停低通气指数AHI为40.1次/h,AHI>40者占51.95%。中位夜间最低血氧饱和度LSaO2为70%,LSaO265%~<85%占45.45%,<65%者占40.69%。血氧饱和度<90%、<85%、<80%分别占监测总时间的百分比(TS90、TS85、TS80)为20.75%、4.53%、1.66%。快动眼相REM睡眠时间占(17.00±7.45)%,非快动眼相(NREM)睡眠时间占(82.96±7.40)%。AHI除与LSaO2、平均夜间血氧饱和度MSaO2呈负相关,与其他指标均呈正相关,其中与氧减饱和指数ODI、每小时氧减时间(简称氧减/h)相关性强。体重指数BMI与LSaO2、MSaO2呈负相关,与AHI、ODI、最大氧减幅度LDD呈正相关。LSaO2与呼吸暂停最长时间、低通气最长时间、TS90、TS85、TS80均呈负相关。结论:男性青年OSAHS患者超重肥胖率高,存在睡眠结构紊乱。
Objective: To analyze the characteristics of polysomnography in the young male patients with obstruc- tive sleep apnea hypopnea syndrome(OSAHS). Methods: Monitoring results of polysomnography (PSG) from 231 young male OSAHS patients were analyzed to learn relationship among the general conditions, apnea and hypopnea events, oxygen reduction and sleep architecture. Results: The rate of overweight and obesity accounted for 93.07 % and 75.76%, respectively. The median AHI and LSaO2 were 40.1 times/h and 70%, respectively; the patients with AHI over 40 accounted for 51.95% ; LSaOz between 65% and 〈85% accounted for 45.45% and with LSaO2 lower than 65 % accounted for 40.69 %. The percent of the total time with oxygen saturation level〈90%, 〈85 %, 〈80%(TS90,TS85,TSS0) accounted for 20.75%,4.53% ,1. 660%, respectively. REM sleep time was (17.00±7. 45) % ,and non--REM(NREM) sleep time was (82.96i7.40) %. AHI was negatively correlated with LSaOz and MSaO2, while it was positively correlated with the others, and had high correlation with ODI and the time of oxy- gen desaturation in one hour. BMI was negatively correlated with LSaO2 and MSaOz, while it was positively corre- lated with AHI, ODI, the largest oxygen desaturation degree (LDD). LSaO2 was negatively correlated with LA, LH,TS90,TS85,TS80. Conclusion: The young male patients with OSAHS have high overweight and obesity rate and manifest obvious disorder of sleep structural.
