三维运动分析系统在健康人平衡功能质心检测中三种方法一致性的比较 被引量:10

摘要 目的:探讨采用三维运动分析系统检测健康人质心的骶骨标记法、测力台法和节段法的一致性,为骶骨标记法用于临床平衡功能评价提供依据。方法:20例健康中年人自愿参加本研究,分别在双腿站、左腿站和右腿站的体位接受三维运动分析系统中的骶骨标记点(SM)的三维空间数据、测力台压力中心(COP)的数据采集和节段法21个标记点的三维空间数据采集以计算获得质心(COM)位置。使用Bland-Altman法比较SM、COP和COM在不同方向上的一致性,及其平均摆动幅度(MSA)的一致性;使用单因素方差分析比较不同站立姿势上述参数之间的差异;并绘出COM和COP、SM的空间位置关系图。结果:SM与COM在左右方向上一致性较好,COP和COM在前后、左右方向上一致性都好;SM、COP的MSA和COM的MSA比较在各方向上的一致性均良好;站立位时COM位于SM下方16mm、前方154mm、左侧9.3mm处;不同站立位下SM、COP和COM的MSA均有显著性差异。结论:以三维运动分析系统检测人体平衡功能质心时,采用骶骨标记法和测力台法相关参数与节段法相比一致性良好,而且可以反映不同体位下质心变化时MSA的变化,可用于站立位平衡功能的检测。 Objective:To study the consistency of three kinds of measurement methods with three-dimentional motion analysis system in locations of center of mass(COM) for balance function assessment with sacral marker(SM).Method:Twenty healthy adults participated in this study,they completed three times of measurements in standing positions with double feet,left foot and right foot.In measurements,sacral marker method,force plate method and segmental method were used.The data of SM location were captured by three dimentional motion analysis system and center of pressure(COP) was recorded on a force plate.Meanwhile,the data of locations of 21 markers with segmental method were captured and used to calculate the COM location.The Bland-Altman plot was used to compare the consistencies between SM and COM on anterior-posterior(AP),medial-lateral(ML) and vertical axis,and between COP and COM on AP and ML axis.Then the consistencies between mean sway amplitudes(MSA) of SM and COM and between MSAs of COP and COM were also compared.One way ANOVA was used to analysis the differences of MSAs among different standing positions.Result:A strong consistency was found between SM and COM on ML axis and between COP and COM on AP and ML axis.The consistency between MSAs of SM and COP was strong as compared to MSA of COM.The location of COM on standing position is 16mm below,154mm anterior and 9.3mm left to SM.The statistically significant differences were found among MSAs of SM,COP and COM of three different standing positions.Conclusion:When the three-dimentional motion analysis system was used in the measurement of location of center of motion,for balance assessment the consistencies between SM and COM,COP and COM were strong,which indicated that the SM method and force plate method were credible for balance assessment,and could show the changes of MSA in the changes of COM at different standing positions.
