全电视辅助胸腔镜外科手术与电视胸腔镜辅助小切口开胸手术临床疗效的比较研究 被引量:20

摘要 目的比较全电视辅助胸腔镜外科手术(c-VATS)与电视胸腔镜辅助小切口开胸手术(VAMT)围手术期的临床疗效。方法将2010年3月至2011年8月该科完成的最初的60例c-VATS作为c-VATS组,同期完成的48例VAMT作为VAMT组,对两组患者手术时间、术中出血量、肺癌淋巴结清扫数目、术后胸腔引流管保留时间、ICU住院时间、术后镇痛时间及术后住院时间进行比较。结果 c-VATS组与VAMT组比较,患者的手术时间、术中出血量、肺癌淋巴结清扫数目、术后胸腔引流管保留时间的差异均无统计学意义(P>0.05),而c-VATS组患者ICU住院时间、术后镇痛时间及术后住院时间明显少于VAMT组(P<0.05)。结论与VAMT相比,c-VATS的术后疼痛更轻,恢复更快。 Objective To compare the perioperative clinical efficacy of complete video-assisted thoracoscopic surgery(c-VATS) and video-assisted mini-thoracotomy(VAMT).Methods The first 60 cases of c-VATS conducted in the department from March 2010 to August 2011 served as c-VATS group and 48 cases of VAMT conducted in the same period served as VAMT group.The operation duration,intraoperative bleeding,number of lymph node dissection in lung cancer,postoperative retention time of chest tube drainage,duration of ICU stay,days of postoperative analgesia and length of postoperative hospital stay of patients in the two groups were compared.Results Compared with the c-VATS group and VAMT group,operation duration,intraoperative bleeding,number of lymph node dissection in lung cancer and postoperative retention time of chest tube drainage of patients showed no statistically differences(P0.05).The duration of ICU stay,days of postoperative analgesia and length of postoperative hospital stay of patients in c-VATS group were markedly shorter than those in VAMT group(P0.05).Conclusion c-VATS possesses features of less postoperative pain and faster recovery when compared to VAMT.
