摘要 目的探讨3D-CTA在颅内动脉瘤手术夹闭与介入栓塞治疗中价值。方法回顾性分析近4年来我科74例颅内动脉瘤破裂出血进行手术或介入栓塞治疗的情况。结果 3D-CTA的动脉瘤发现率与DSA完全一致,而3D-CTA对动脉瘤的形态、位置、瘤颈、与周围的关系、尤其是与前床突的关系与术中所见完全一致,而且更清晰直观,明显优于DSA。对介入栓塞、手术入路及瘤夹的选择很有帮助。结论 3D-CTA能更好的了解动脉瘤的形态与周围的解剖结构,对确定动脉瘤的介入栓塞与手术方案比DSA有更好的参考价值。 Objective To study the value of three-dimensional computed tomographic angiography (3D-CTA) in surgical treatment and interventional embolotherapy of intracranial aneurysm. Method Retrospective analysis of 74 cases with rupture of intracranial aneurysm which was treated by surgical treatment or interventional embolism in our department for past 4 years. Results The detection rate of aneurysm with 3D-CTA were completely consistent with the DSA, and the aneurysm form, location, aneurysm neck, surrounding relations, especially relations with the anterior clinoid process in 3D-CTA were consistent with the intraoperative findings. The image of 3D-CTA was more clear and intuitive compared with DSA. 3D-CTA is helpful to the interventional embolism, surgical approach and the choice of aneurysm clip. Conclusion The reference value of 3D-CTA is better than DSA in understanding the form of aneurysm and its surrounding anatomy which was useful to choice therapeutic scheme in intracranial aneurysm.
