密度梯度离心法与直接上游法在宫腔内人工授精中的应用效果分析 被引量:2

摘要 目的探讨密度梯度离心法与直接上游法两种方法优化处理精液行宫腔内人工授精对精子动态参数和受孕率的影响。方法对116例患者共254个IUI治疗周期作回顾分析,将254个周期随机分为A、B两组,两组精子的动态参数无显著性差异,A组采用直接上游法,B组采用密度梯度离心法优化处理精液后行宫腔内人工授精,比较两种方法的精子检测结果与妊娠结局。结果两种方法处理前后精子密度、活率、活力(a+b)、畸形率的差异均有显著性(P<0.05);两种方法处理后精子密度的差异有显著性(P<0.05);密度梯度离心法高于直接上游法,但活率、活力、畸形率的差异无显著性(P>0.05),两组间的临床妊娠率及流产率的差异也无显著性(P>0.05)。结论两种方法处理精液均可改善精子动态参数,但行宫腔内人工授精得到的临床妊娠率差异无显著性(P>0.05)。 Objective To discuss the effects of density gradient eentrifugation and direct swimup sperm preparalion in intrau- terine insemination (IUI) on semen mobility parameters and pregnancy rate. Methods A retrospective analysis was made to 254 cycles in 116 cases; the cycles were divided into 2 groups: Group A and Group B and there existed no obvious difference of the semen mobihty parameters between the 2 groups ; the direct swim - up sperm preparation was applied in Group A while den- sity gradient centrifugation was applied in Group B in IUI, and the results of sperm detection and pregnancy of the 2 methods were compared. Result The sperm concentration, motility rate, ( a + b ) and deformity rate in the 2 groups after treatment were of obvious difference from those before treatment ( P 〈 0.05 ) ; There existed an obvious difference of sperm concentration after treatment between Group A and Group B ( P 〈 0.05 ), the sperm concentration in Group B was higher than that in Group A, no difference in motility rate, ( a + b ) and the deformity rate was found ( P 〉 0.05 ) and no difference in pregnancy rate and abor- tion rate ( P 〉0. 05 ). Conclusions Both methods can improve the sperm mobility parameters and they have no obvious difference from each other in clinical pregnancy rate of IUI.
