摘要
目的观察腔内微波热疗十同步放化疗治疗中晚期宫颈癌的临床疗效。方法 2000年2月至2001年2月我院确诊的Ⅱb~Ⅲ宫颈癌患者的61者均采用8MV-X线治疗,全盆剂量38~40Gy,宫旁剂量10~18Gy,192Ir-HDR腔内放疗剂量30~36Gy;放疗同时用顺铂＋5-Fu联合化疗;顺铂20mg/m2第1～5天,5-Fu 750 mg/m2第1～5天,4周重复一次,共2～4次;热疗采用915MHz微波热疗仪,阴道内辐射器加温42～47℃,45min/次,1～2次/周,共7～10次,放疗前或放疗后1h内进行。结果完全缓解（CR）48例（78.69%）,部分缓解（PR）9例（14.75%）;全组病例3、5年生存率分别为80.3%和72.9%。结论热疗十同步放化疗能增加宫颈、阴道上1/3恶性肿瘤的敏感性,提高宫颈癌患者的生存率,临床疗效明显。
Objective To evaluate the therapeutic result of carcinoma of uterine cervix treated by chemoradiation combined with hyperthermia.Methods The 61 patients of stage Ⅱb～ Ⅲ cervical cercinoma were treated from Febrary 2003 to Febrary 2006.A midplane dose of 38-40 Gy to the whole pelvis and 10-18Gy to the parametrical areas were given using 8 MV X rays,combined 30~36Gy with 192Ir HDR intracavitary brachytherapy.Each patient was given DDP with combined chemotherapy at the dose of 20 mg/m2.From the firstday the fifth day,5-fu 750mg/m2,the treatment cycle was performed once every 4 weeks for 2～4 times.Hyperthermia was given an hour before or after radiotherapy.The temperature was increased and maintained at 42-47 ℃ for 45 minutes with 915Mhz microwaves.Hyperthermia was given one or two times a week for 7～10 fractions.Results The complete remission of the group was 78.69%,the partial remission was 14.75%.A11 patients were followed up over five years.The 3～ and ～5 year survival rates 80.3%and 72.1%.Conclusion Increasing the radiosensitivity of cervix and 1/3 upper vagin,hyperthermia combined with chemoradiation is able to enhance the local control rate and survival rate for the patients with carcinoma of uterine cervix.
