摘要 目的探索牙髓炎保存活髓的方法。方法将20只成功建立实验性牙髓炎模型的新西兰大白兔,按时间分为1、3、5、7 d 4组;每只兔采用自身对照的方法,随机将两颗下颌前牙分为实验组和对照组,实验组在牙颈部钻2个引流孔,分别插入制备好的5号小儿头皮静脉针,头皮针周围用玻璃离子密封,再分别接5 mL注射器,1支用于抽负压,另1支向髓腔内注入抗生素液,冲洗结束后,头皮针两端胶管用酒精棉球填塞,头皮针作为引流管保留;对照组使用碘仿糊剂处理。分别于1、3、5、7 d处死兔子,收集牙齿标本,显微镜下观察炎症控制情况。结果实验组第1天5例中炎症4例,愈合1例;对照组5例中炎症5例,愈合0例。实验组第3天5例中愈合5例,炎症0例;对照组5例中愈合2例,炎症3例。实验组第5天5例中愈合5例,炎症0例;对照组5例中愈合4例,炎症1例。实验组、对照组第7天全部愈合。结论牙髓炎进行牙颈部钻孔引流能有效保存活髓。 [Objective] To search for a vital pulp preservative method in experimental pulpitis. [Methods] 20 rabbits were selected for this study, they were divided into four groups: 1, 3, 5, 7 day according to experimental interval; every rabbit mandible anterior teeth were divided into experiinental groups and control groups on the basis of own control randomly. Experimental pulpitis animal model was alade in rabbits tooth then two pinholes were drilled in middle 1/3 dental cervix of buecal labial surface of crown of tooth, infant transfusion needle processed was inserted into pinhole and it was sealed up with glaslonomer then two 5 mL injection units were contacted with transfusion needle, one under pressured the pulp chamber, another infuse antibiotics to pulp chamber, after treatment, it was plugged with alcohol cotton ball, and reserve as drainage tube. Rabbits were executed in 1, 3, 5, 7 day, inflammation was observed in microscope respectively. [Resuits] 4 inflammations, 1 healing out of 5 cases in experimental group, 5 inflammations, 0 healing out of 5cases in control group in first postoperative day; 0 inflammation, 5 healings out of 5 cases in experimental group, 3 inflammations, 2 healings out of 5eases in control group in third postoperative day; 0 inflammation, 5 healings out of 5 cases in experimental group, 1 inflammation, 4 healings out of 5 cases in control group in fifth postoperative day; 0 inflammation, 5 healings out of 5 eases in experimental group, 0 inflammation, 5 healings out of 5 cases in control group in seventh postoperative day. [ Conclusion ] Two pinholes drilled in middle 1/3 dental cervix of buecal labial surface of crown of tooth can preserve vital pulp efficiently.
