控制性肺膨胀对急性呼吸窘迫综合征开放吸痰后血清炎症相关细胞因子的影响

摘要 目的:探讨控制性肺膨胀(SI)对Beagle犬急性呼吸窘迫综合征(ARDS)开放吸痰后炎症相关细胞因子的影响。方法:选择12只Beagle犬,利用油酸建立犬ARDS模型,稳定30min后随机进行两组吸痰,SI组吸痰后立即予以一次SI。非SI组吸痰后不予SI。利用ELISA方法检测非SI组及SI组在使用油酸成模前、成模稳定后、吸痰后1h和4h时间点血清中肿瘤坏死因子α(TNF-α)(pg/ml)、白介素1β(IL-1β)(pg/ml)、白介素6(IL-6)(pg/ml)、白介素10(IL-10)(pg/ml)的水平。结果:Beagle犬经油酸成模后,各类炎症相关细胞因子特别是炎性细胞因子TNF-α、IL-1β、IL-6,及抑炎细胞因子IL-10水平较使用油酸成模前均显著升高(P<0.01);与此同时,与单纯吸痰(非SI组)相比而经吸痰和控制性肺膨胀(SI组)后,上述细胞因子均有下降(P<0.05),并且这种作用随着吸痰后通气时间的延长而延长。结论:SI能改善ARDS开放吸痰后肺的氧合指标,改善肺顺应性的分子基础可能是改善组织(特别是心肺组织)炎症程度。 Objective: To evaluate the treatment value of sustains inflation (SI) in inflammation-related cytokines of serum after open endotrachel suctioning in acute respiratory distress syndrome (ARDS). Methods: Chose twelve Beagle canines, oleic acid-induced ARDS canine models were randomly divided into two groups of open endotrachel suctioning after stabilizing for 30 min. SI-group: used SI in ARDS Beagle dog after open endotrachel suctioning. Non-SI-group: did not use SI in ARDS Beagle dog after open endotrachel suctioning. Inflammation-related cytokines of serum such as TNF-α, IL-1β, IL-6 and IL-10 were measured at 4 time points (before OA, OA exposure, open endotrachel suctioning 1 h, open endotrachel suctioning 4 h ) by ELISA assay. Results: All inflammation-related cytokines were increased after open endotrachel suctioning at the 3 time points in the two groups, including the anti-inflammation cytokine TNF-α, IL-1β, IL-6, IL-10 (P0.01). However, there were less augment in SI-group inflammation-related cytokines compared with Non-SI-group (P0.05). Furthermore, the less augment was time-dependent decrease. Conclusion: The molecular mechanism of effect of SI in better oxygenation and lung compliance is attributed to improve the degree of inflammation.
