影响子宫肌瘤患者接受超声消融治疗相关因素的初步分析 被引量:7

摘要 目的:分析子宫肌瘤患者接受超声消融治疗的情况及未接受超声消融治疗的原因。方法:回顾性分析四川省遂宁中心医院及重庆市涪陵区中心医院妇产科超声消融治疗中心从2010年7月到2011年5月接受咨询的729例患者。按照国家食品药品监督管理局通过的标准进行初步筛选后,再进行机载超声模拟定位、盆腔核磁共振成像检查及临床检查以进一步排除不适合超声消融治疗的患者。结果:33.5%(244/729)的患者最终接受了超声消融治疗。未接受超声消融治疗的原因包括:治疗意愿不强(36.2%;264/729);不适合超声消融治疗(12.5%;91/729);选择妇科手术(12.3%;90/729);超声消融设备局限(1.6%;12/729);经费问题(3.3%;24/729)。结论:目前接受超声消融治疗的比例不高,要提高患者的接受程度还有许多工作要做,包括提高超声消融的知晓度。 Objective:To investigate eligibility and accessibility of high intensity focused ultrasound(HIFU) treatment for patients with symptomatic uterine fibroids.Methods:729 patients referred to HIFU treatment for uterine fibroids from two new HIFU centers were retrospectively analyzed.The patients were first evaluated following the eligibility criteria of the State Food and Drug Administration of China.Then,the eligible patients underwent simulation,pelvic MRI and clinical examination to determine suitability for the procedure.Results:33.5% of the patients(244/729) accepted HIFU treatment.The main reasons that patients didn't take HIFU treatment were as follows:not desire HIFU treatment(36.2%,264/729);not suitable for HIFU(12.5%,91/729);choosing gynecologic surgery(12.3%,90/729);ultrasound ablation equipment limitations(1.6%,12/729);and financial difficulties(3.3%;24/729).Conclusion:The proportion of patients who accepted HIFU ablation is still low.We need to do more work in improving the accessibility of this technique.
