超声引导下微波凝固离体猪肾的实验研究

摘要 目的通过研究超声引导下微波对离体猪肾的凝固作用,为超声引导下微波凝固治疗肾脏病变的可行性及安全性提供依据。方法在超声引导下,采用KY-2000型微波治疗仪和水循环内冷式微波天线以不同作用功率、时间对离体猪肾进行微波凝固,观察微波凝固过程,分析凝固灶范围与功率和时间的关系。结果超声可清晰显示微波针在肾实质内的位置。在正常离体猪肾中微波凝固灶呈椭球体,功率一定时,凝固灶最大断面长径、宽径与作用时间近似线性相关,宽径增幅大于长径增幅;作用时间一定时,凝固灶长径、宽径与功率近似线性相关,增幅以40-60w明显。结论超声引导下微波凝固离体猪肾定位准确,凝固过程可实时观察,在肾实质内形成凝固灶范围可控,本研究结果可为进一步在活体中的应用提供客观实验数据。 Objective To investigate the effects of microwave coagulation guided by ultrasonography on porcine kidney in vitro in order to investigate the possibility and safety of microwave coagulation for kidney diseases. Methods Guided by ultrasonography, microwave coagulations were performed on vitro porcine kidneys with differential irradiation powers and durations. The relationship between the effective size of lesion and the power and duration was analyzed. Results The position of the microwave antenna was showen clearly by ultrasound during the coagalation. The effective areas of lesions were all ellipsoid. When treated with a same power, the maximum longitudinal diameter and transverse diameter of effective lesions were linearly related with the acting duration, and the increase of transverse diameter were larger than that of the longitudinal diameter. When treated with a same duration, the maximum longitudinal diameter and transverse diameter of effective lesions were linearly related with the acting power, and the optimal power was 40W to 60W. Conclusion Microwave coagulation in porcine kidney can be accurately guided in real time by ultrasonography. The effective area of microwave coagulation in kidneys is in control.
