脑电生物反馈治疗对痉挛型脑瘫患儿脑功能的改善作用 被引量:12

摘要 目的:探讨痉挛型脑瘫患儿经脑电生物反馈(EEGBF)治疗后脑功能的改善情况。方法:选取符合诊断分型标准的痉挛型脑瘫患儿60例,平均年龄7.32岁;按照性别、年龄、病情严重程度及入院顺序,采用随机方法分成观察组与对照组各30例。对照组采用综合康复治疗方法,观察组除采用与对照组相同方法外,加用脑电生物反馈治疗,每周训练5次,疗程为3个月。采集观察组和对照组患儿脑电波,包括θ波、β波、α波、SMR波、θ/β比值、α/SMR比值,经仪器自身分析处理测评干预前后患儿脑电波的变化。同时对两组干预前、后进行中国比内智力测试和注意力变量检测。结果:治疗后,观察组患儿θ波均值明显下降(P<0.05),β波均值明显升高(P<0.05),θ/β功率比值明显下降(P<0.05);α波均值明显下降(P<0.05),SMR波均值明显升高(P<0.05),α/SMR功率比值明显下降(P<0.05)。对照组θ波均值治疗后与治疗前比较明显降低(P<0.05),β波均值、θ/β功率比值、α波均值、SMR波均值及α/SMR功率比值与治疗前比较差异不显著(P均>0.05)。治疗后,观察组智力水平较治疗前显著提高(P<0.001);注意力变量各观察指标分值与治疗前比较差异显著,观察组的疗效优于对照组。结论:脑功能生物反馈治疗可有效改善痉挛型脑瘫患儿的脑功能。 Objective: To explore the effect of electroencephalographic biofeedback(EEGBF) technique on brain function of children with spastic cerebral palsy. Method:Sixty children with spastic cerebral palsy were enrolled. According to their gender, age, degree of illness and order of admission, the children were randomly divided into 2 groups. Everyday, 30 children in observation group received routine rehabilitation, and EEGBF therapy; 30 children in control group only received routine reha- bilitation without EEGBF. The course of treatment lasted for 3 months. Electroencephalogram was used to deteet the changes of 0 waves, [3 waves, c~ waves,SMR waves, c~/SMR power ratio, 0/13 power ratio. Before and after in- tervention, Chinese-Binet intelligence scale(CBIS) and the test of variables of attention(TOVA) were tested. Result: Observation group: After 3 month-EEGBF treatment, the children's 0 waves reduced markedly (P〈0.05), 13 waves increased significantly(P〈0.05), 0/13 power ratio decreased obviously(P〈0.05); ct waves decreased obvi- ously(P〈0.05), SMR wave increased significantly(P〈0.05), c~/SMR power ratio decreased obviously(P〈0.05). Controlgroup: comparing with those waves before treatment, 0 waves decreased obviously (P〈O.05), [3 waves, 0/13 pow- er ratio, oL waves, SMR waves and ot/SMR power ratio showed no significant difference (P〉0.05). After treat- ment, in observation group the intelligential elevated significantly(P〈0.O01), and scores of all i^dexes of TOVA showed significant differences than those before treatment, Namely the curative effect of observation group after treatment was better than control group.
