摘要
目的 观察自体真皮移植术修复甲床缺损的临床效果及可行性.方法 2009年8月至2010年6月,对24例外伤性甲床缺损,随机分成A、B两组,每组12例.甲床缺损面积为2mm×4 mm～5 mm×8 mm.A组采用自体真皮移植术修复,B组采用自体甲床移植术修复.术后随访观察两组指甲生长情况.结果 术后2周拆除加压包后,A组有1例出现部分坏死,经换药后愈合；余23例创面全部愈合.随访时间为3～12个月,术后3个月患指指甲均能长出覆盖甲床,A组3例、B组1例新生指甲不平滑,外观欠佳.A组供皮区无病例发生并发症；B组供趾甲床不平滑2例,外观欠佳.24例患指均无疼痛,捏持和握力不受影响.结论 自体真皮移植术疗效满意,取材方便,对供区影响小,可作为传统自体甲床移植的替代治疗方案.
Objective To explore the feasibility and evaluate the clinical outcomes of repairing nail bed defect by autologous dermis grafts.Methods Twenty-four cases of traumatic nail bed lesions treated between August 2009 and June 2010 were randomly divided into group A and B,with 12 cases each.The size of the defects ranged from 2 mm × 4 mm to 5 mm × 8 mm.In group A the nail bed defect was repaired by autologous dermis grafts.Autologous nail bed transplantation was done to reconstruct the defects in group B.Nail growth was observed and compared in both groups.Results Compressive dressing was removed 2 weeks postoperatively.Except the partial necrosis observed in one case of group A,all grafts survived and wounds healed uneventfully.Postoperative follow-up period ranged from 3 to 12 months.Nails could grow to cover nail bed in 3 months.Unevenness and less optimal appearance of nails were seen in 3 cases of group A and 1 case of group B.There were no donor site complications in group A.Unevenness of the donor nail bed was observed in 2 cases of group B.There was no pain,and pinch function and gripping force were unaffected in all 24 cases of both groups.Condusion Autologous dermis graft is easy to harvest and causes minimal donor site morbidity.It leads to satisfactory clinical results and thus can replace the conventional autologous nail bed transplantation.
