脂肪乳对急性有机磷中毒大鼠肾损伤治疗作用的实验研究 被引量:6

摘要 目的探讨急性有机磷中毒所致的肾损伤程度分析,脂肪乳对急性有机磷所致肾损伤的保护作用。方法取SD封闭大鼠48只,随机分为A,B,C,D 4组,予敌敌畏11mg/kg腹腔注射染毒。A组为盐水对照组,染毒后予生理盐水(5mL/kg)静脉注射;B组为脂肪乳对照组,染毒后予20%脂肪乳(5mL/kg)静脉注射;C组为常规治疗组,染毒后立即予阿托品(10mg/kg)和氯解磷定(40mg/kg)肌肉注射;D组为脂肪乳治疗组,在C组的基础上联合应用20%脂肪乳(5mL/kg)静脉注射。分别于染毒前,染毒后12h取眶下血测血清肌酐、尿素;染毒后24h取肾脏行电镜检查。结果敌敌畏染毒后较染毒前肌酐、尿素升高明显,差异有统计学意义(P<0.05);染毒后12h,C、D组较A、B组,D组较C组,B组较A组肌酐、尿素差异有统计学意义(P<0.05);肾脏组织电镜检查示脂肪乳辅助治疗组较其他3组肾脏组织改变明显减轻。结论急性有机磷中毒对肾脏造成严重损害,脂肪乳联合解磷定和阿托品治疗,可以保护肾脏。 Objective To explore kidney injury induced by acute organophosphorus poisoning and therapeutical effect of lipid emulsion to acute kidney injury induced by acute organophosphorus poisoning in rats.Methods Forty-eight sealant grade Sprague Dawley(SD) rats,were randomly divided into four groups A,B,C,D: saline control group,lipid emulsion control group,the conventional therapy group and lipid emulsion administration group.11 mg/kg body weight dichlorvos(DDVP) was administrated through peritoneal injection in all rats,and then the rats in 4 groups were respectively treated: with nomal saline(5 mL/kg) intravenous injection,lipid emulsion(5 mL/kg)intravenous injection,atropine(5 mg/kg) and pralidoxime chloride(40 mg/kg) intramuscular injection,combined use of lipid emulsion(5 mL/kg)with atropine and pralidoxime chloride after administrating DDVP peritoneal injection.The creatinine,urea in blood was detected before and 12 h after poisoning in DDVP.The pathological changes of the kidney tissue were observed within following 24 h.Results In groups A,B,C,D the creatinine,urea in blood were significantly different compared with before poisoning in DDVP;In groups C,D the creatinine,urea in blood was significantly different compared with group A,B as D compared with C and B compared with A 12 h after poisoning in DDVP(P〈0.05);electron microscopic finding should combined use of lipid emulsion with atropine and pralidoxime chloride obviously improve the kidney pathologies induced by poisoning in DDVP.Conclusion Acute organophosphorus poisoning induce kidney injury,lipid emulsion combined lipid emulsion with atropine and pralidoxime chloride can protect kidney injury induced by acute organophosphorus poisoning.
