摘要 目的比较I-gel喉罩与普通喉罩用于全麻乳腺手术中的气道管理效果。方法全麻下行乳腺肿物切除术患者60例,ASAⅠ或Ⅱ级,随机均分为I-gel喉罩组(I组)和普通喉罩组(S组)。记录首次插入成功率、插入时间、气道密封压、纤维支气管镜检查分级情况、苏醒时间、拔出喉罩时间及喉罩插入成功后15、30、45、60min时HR、BP、SpO2、PETCO2及气道峰压(Ppeak)值;记录并发症的发生情况。结果 I组首次插入成功率为90%,高于S组的76%(P<0.05);插入时间为(15.2±5.6)s,短于S组的(26.5±8.3)s(P<0.05);气道密封压为(27.6±4.7)cmH2O,高于S组的(23.8±4.5)cmH2O(P<0.05);纤维支气管镜评级优于S组(P<0.05)。两组患者苏醒时间、拔除喉罩时间和各时点HR、BP、SpO2、PETCO2及Ppeak差异无统计学意义;两组患者拔除喉罩后并发症的发生率差异无统计学意义。结论 I-gel喉罩用于全麻乳腺手术气道管理的效果优于普通喉罩。 Objective To compare the application of the I-gel laryngeal mask and the common laryngeal mask in breast surgery with general anesthesia. Methods Sixty patients, ASA I or II, undergoing the breast surgery with general anesthesia were randomized into two groups: I-gel group (group I) and common group (group S). Achievement ratio of first time insertion, inserting time, airway sealing pressure, classification after bronchoscopy of fiberoptic bronchoscope, wake time and extubation time were recorded. HR, BP, SpO2, PErCO2 and peak airway pressure (Ppeak) were monitored 15, 30, 45 and 60 min after insertion. The complications were also recorded. Results Compared with group S, achievement ratio(76% vs 90%), airway sealing pressure [(23.8±4.5)cm H20 vs (27.6±4.7) cm H2O and classification were better and the inserting time [(26.5±8.3)s vs (15.2±2.6)s] were shorter in group I (P〈0. 05). There was no significant difference in wake time, extubation time, and HR, BP, SpO2 and Ppeak and complications in both groups. Conclusion Application of the I-gel laryngeal mask in airway management during breast surgery with general anesthesia is better than the common laryngeal mask.
