摘要 背景与目的:颅内血管周细胞瘤影像表现易与脑膜瘤混淆,本文探讨颅内血管周细胞瘤(hemangiopericytoma,HPC)的影像学表现,并复习相关文献,以提高诊断水平。方法:回顾性分析经手术病理证实的的颅内HPC 12例。12例均行MRI平扫和增强扫描。结果:12例颅内HPC的MRI特征:肿瘤呈不规则形或分叶状(9例,75%),肿瘤呈窄基底附着于硬膜(7例,58.3%),瘤内迂曲血管流空影(7例,58.3%),瘤内见囊泡状T1低信号和T2高信号(8例,66.7%);DWI呈高信号(8例,66.7%);肿瘤强化呈不均匀明显强化(8例,66.7%)。结论:颅内血管周细胞瘤影像学表现具有一定特征性,肿瘤呈不规则形或分叶状、瘤内血管流空信号、DWI呈高信号、明显不均匀强化及肿瘤以窄基底附着于硬膜是诊断颅内HPC重要征象。 BACKGROUND & OBJECTIVE: The imaging features of intracranial hemangiopericytoma were investigated and related literatures were reviewed in order to improve the level of diagnosis, due to imaging manifestations of intracranial hemangiopericytoma were confused with meningioma. MATERIALS & METHODS: 12 cases of intraeranial hemangiopericytoma confimed by operation and pathology were analyzed. Pre and post contrast MRI scanning were performed. RESULTS: MRI features of 12 cases of intracranial HPC were found as irregular or muhilobulated shape in 9 cases (75%), narrow-based dural attachment in 7 cases (58.3%), internal serpentine signal voids in 7 cases (58.3%), vesicle-like tumor with low signal in T1WI and high signal in T2WI in 8 cases (66.7%), high hyper-intensity on DWI in 8 cases (66.7%), and strongly heterogeneous enhancement in 8 cases (66.7%), repeetively. CONCLUSION: Intracranial Hemangiopericytomas have typical MRI features, which may facilitate pre-operative diagnosis.
