摘要 介绍了一种基于微机的心房晚电位的检测与分析系统 通过对R波触发和P波触发的信号平均心电图方法的讨论和比较 ,获得了一套实用可行的P波提取方法 标准P波宽度、空间向量的总P波宽度和最后 2 0ms均方根电压是一些与房颤关联的诊断参数 详细的介绍了本系统获取这些参数的算法 ,并给出了一套辅助临床诊断的参考指标 初步使用的情况表明 ,这个系统的检测和分析方法是有效的 ,在临床上可以对房颤和房扑进行预测 ,目前国内对心房晚电位的研究比较少 。 This paper introduces an Atrial Late Potentials (ALP) detecting and analyzing system based on micro computer. The system comprises of real time data acquisition module and digital signal analyzing program. Prolongation of atrial conduction is expected to lead to prolongation of the body surface P wave, and increased fractionation within the atrial signal may cause an increase in high frequency low amplitude signals. Therefore ALP is considered to be a useful hint in the prediction of Paroxysmal Atrial Fibrillation (PAF). But the relatively low amplitude of P wave and the presence of other cardiac electrical signals or noises pose a challenging problem in terms of extracting clear signals. The main purpose of this research is to develop effective and reliable algorithms for detecting ALP and to define robust indicators appropriate for clinical diagnosis of PAF. After elaborated comparisons made between the R wave triggered Signal Averaged Electrocardiogram (SAECG) and the P wave triggered SAECG, a proper method for the acquisition of P wave is proposed. Analysis techniques, including standard P wave duration, total P wave vector duration and last 20?ms root mean square (RMS) voltage are strongly associated with the presence of PAF. Total P wave vector duration is significantly longer in the patients with PAF than in the control group while the last 20?ms RMS voltage significantly decreased in the subject group. Algorithms for evaluating these indicators are introduced in detail and a series of normal values are proposed in this paper. In conclusion, a result, total P wave vector duration 130?ms or more and last 20?ms RMS voltage 3.5?μv or less, suggests the occurrence of ALP at the end of P wave. Noticing that different methods of processing produce unique results and have a unique range of values defined as normal, the validity of the criteria is limited to this system. Tryouts suggest that the detecting and analyzing methods applied in this system are effective in identifying patients with PAF or atrial
